Special Procedures Nurse - Care Transition Clinic, Part-Time, Days

Under the direction of a faculty sponsor and in collaboration with the program support team, coordinates population health care needs for patients and potential patients in the community across the care continuum.  Acts as a clinical liaison for patients and potential patients, across medical service, clinical departments, and locations. Assesses clinical, emotional, spiritual, psychosocial, financial, and other patient needs.  Evaluates and manages patient care gaps and assists in the integration of care across departments to address those gaps.

 

Under the direction of an Attending Physician or a licensed independent practitioner, the Special Procedures Nurse RN provides technically complex and high risk nursing care services to patients which may include one or more of the following categories:  sedation, intravenous infusions, complex procedures, e.g., lumbar puncture, cardiac catherization; This may also include specialized monitoring during procedure and recovery phase; The RN may have Charge Nurse responsibility for his/her assigned unit.

Recognizes patient conditions requiring immediate actions, initiates appropriate life supporting interventions while communicating appropriately with physicians.
  • Maintains nursing equipment and work areas in proper working condition; reports malfunctioning equipment or housekeeping issues to the appropriate person. Maintains a neat and clean work area with equipment. Maintains adequate inventory control of all necessary supplies. Emergency equipment maintained with adequate availability, good working order and restocks as needed with a fiscal awareness and accountability.
  • Interfaces with customers, specifically patients, families, visitors, and UCMC staff in a professional, positive and accommodating manner; demonstrates knowledge of AIDET principles and employs in all situations; demonstrates flexibility in accommodating special needs of customers whenever possible.
  • Performs all required Quality Assurance testing as indicated or as required by UCMC
  • Rotates to clinical areas as directed to assist in safe staffing ratios; utilizes resources to safely transport sedated patients to an appropriate  or designated area within the UCMC

  • Bachelor of Science in Nursing preferred; experience may be considered in lieu of degree in addition to 5+ years clinical experience
