The Hospital at Home nurse is responsible for providing high-quality, at-home acute care. The Hospital at Home nurse will treat various conditions and patient populations within the framework of UChicago Medicine’s Acute Hospital Care at Home model. The position requires acute care competencies, along with an ability to work in an unstructured, at-home setting. In addition to providing clinical care, Hospital at Home nurses will be able to explain the unique care plan, technologies, and processes of the acute care at home model to patients and caregivers. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires.

Essential Job Functions:

Utilize monitoring, assessment; planning, intervention and critical thinking skills to care for acute care patient populations in the home.
Navigate patients and families across the continuum of the UChicago Medicine Hospital at Home phases of care.
Monitor and respond to biometric data per UChicago Medicine Hospital at Home protocols.
Conduct home and audio/visual patient care visits as directed by the care team and/or protocol.
Ensure all interventions in the patient's care plan are implemented efficiently with a patient centered approach.
Leverage the resources and expertise of the UChicago Medicine partners and supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation.
Adhere to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care.
Promote professional practice and a culture of safety; willingness to engage in process improvement efforts.
Provide protocol driven assessment, care coordination and interventions as appropriate informed by UChicago Medicine Hospital at Home protocols for diverse patient populations.
Exhibit sound clinical decision making and critical thinking skills.
Maintain and practice in accordance with nursing guidelines per state and hospital regulations.
Facilitate admissions to Hospital at Home utilizing UChicago Medicine Hospital at Home guidelines.
Utilize nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems.
Share expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise.
Support research efforts to advance knowledge and promote evidence-based practice.
Perform other duties and responsibilities as assigned by the Nurse Manager.

Qualifications:

Associate degree required
2 years of Registered Nurse experience in an acute care setting. Home health experience preferred.
Valid driver’s license, reliable vehicle and proof of vehicle liability insurance.
Must be able to communicate effectively in English.

Licensure:

Current RN Illinois license.
Current BLS certification upon hire, or completion within 30 days of hire required.

The Family Connects model is an evidence-based and successfully demonstrated program that connects parents of newborns to the community resources they need through postpartum nurse home visits. The model was developed in 2008 through a partnership with Duke University, the Center for Child & Family Health (CCFH), and the Durham County Health Department. As a community-based program, Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parents’ needs, and helps to identify community services or resources that can meet those needs. In addition, through this connection to resources, Family Connects helps to identify gaps in critical community-wide resources with the goal of working toward increasing needed services locally. A Family Connects Home Visiting Nurse provides home visiting to all families with newborns in Chicago. The goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The Home Visiting Nurse will work in a team environment of clinical and non-clinical staff in order to schedule visits, and support families in the community using the evidence-based Family Connects home visiting protocol. In addition to home visiting responsibilities, the Home Visiting Nurse will support the Prenatal Education Department by teaching a minimum of one in-person prenatal education class per week, as assigned. This responsibility includes approximately two hours of classroom instruction and one hour of preparation time per class. The nurse will deliver evidence-based education on topics such as pregnancy, labor and birth, postpartum recovery, newborn care, breastfeeding, and other maternal-child health topics, while serving as a clinical resource for expectant and new families.
