Responsibilities*

Under Physician Supervision, Directs And Coordinates Care As a Primary Service For Hospitalized Patients With Advanced Heart Failure, Pre And Post-heart Transplant, And Medical Management Of Left Ventricular Assist Devices (LVAD). Management Of This Patient Population Includes Cardiology And Internal Medicine. The NP/PA Will

Complete detailed histories and physicals, daily progress notes, streamline the patient transfer process between levels of care, and discharge orders/documentation.
Collect and interpret reports from diagnostic lab tests, radiographic procedures, clinical studies.
Order tests/prescribe medications.
Present pertinent data to the medical staff and assist in maintaining continuity of care throughout Cardiovascular Medicine, including thorough and accurate hand-offs.
Patient/family education on heart failure, heart transplant, and LVAD.
Procedures include point-of-care ultrasound for physical assessment, placement of wound vacs, and placing NG tubes.
Confer with attending physicians and collaborate with other care providers/consultants to coordinate and communicate clinical care needs both inpatient and outpatient, and internal/external care providers.
Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians and the Heart Failure/Transplant/MVAD Outpatient Teams.
Participation in quality assurance activities including Lean Projects with the Heart Failure/Transplant/MVAD Team.
Perform other related duties as required.
Maintain an in-depth knowledge of medical principles, practices, standards, and techniques in accordance with Michigan Medicine policies and procedures, Medical Staff Bylaws, and State and Federal laws/regulations.
Participate in activities that promote professional growth and self-development.
Adhere to all Michigan Medicine dress codes, personnel, and safety policies.
Perform other related duties as required to assure patient safety and satisfaction utilizing service excellence and Patient Family Centered Care in daily practice.

Required Qualifications*

Nurse Practitioners

Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
Certification as an Acute Care Adult Gerontology Nurse Practitioner or Acute Care NP.
Graduate of an accredited Nurse Practitioner program or who has successfully completed all required Nurse Practitioner program coursework at an accredited Nurse Practitioner program to qualify to take their specialty certification exam.
Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
Current DEA registration required.
AHA BLS and ACLS required.

Physician Assistants

Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
Certification by the National Commission on Certification for Physician Assistants (NCCPA).
Current license to practice as a Physician Assistant in the State of Michigan.
State of Michigan Controlled Substance License Required.
Current DEA registration required.
AHA BLS and ACLS required.

Desired Qualifications*

1 year or more of full time Advance Practice Provider (APP) experience with 50% or more clinical time spent working with Heart Failure patients, Pre/Post-Heart Transplant patients, and/or LVAD patients within the last five years.
Recent (within the last five years) APP experience working adult cardiology patients in an inpatient setting.
APP experience prescribing and managing immunosuppression medications.
1 year or more APP experience rounding on an inpatient internal medicine or hospitalist service caring for acutely ill patients on telemetry, step-down, or ICU settings.
One or more of the following certifications in Heart Failure: CHFN-K, HF-Cert, or OMT-HF
One or more of the following certifications in cardiology: CCKE, ABCL, or CHC
Proficiency reading 12 lead ECGs.
Ability to demonstrate good computer skills including computer documentation and order entry.
Demonstrates ability to function independently and as a team member, working collaboratively with others.
Demonstrated clinical leadership and experience with coordinating and managing complex, patients and providing safe hand offs to the next provider.
Demonstrated expertise in facilitating the educational growth of others through previous clinical instructional roles.
Demonstrated ability to collaborate clinical care ideas across multiple patient care services and to communicate effectively as a patient advocate.
Demonstrated commitment to carrying out professional responsibilities, adherence to ethical principles, sensitivity to diversity, and sharing knowledge with team members.
Demonstrated clinical leadership through the coordination and management of complex patients, facilitating safe and seamless transitions of care with effective verbal and written communication, rigorous attention to detail, and thorough provider handoffs.

This clinician may initially obtain mentored experience in Sleep Medicine at the Sleep Clinic, and then work in collaboration with physicians in the Sleep Clinic. Responsibilities* Expectations for this position would include providing direct care to patients in the Sleep Disorders Clinic, provide support to the physicians in clinic, handle higher level medical issues, and contribute to the overall continuity of patients' care. This clinician would provide clinical leadership in practice, staff development and research. Participate in student, physician, and staff education and participate in ongoing quality improvement initiatives. Candidate would be expected to work with clinical staff for scheduling and triage issues as well as supporting clinic nurse in the provision of patient care as needed with the service. Required Qualifications* Required Qualifications (NP) Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan. Current specialty certification to practice as a Adult Gero-Acute Care Nurse Practitioner or Family Nurse Practitioner Graduate of an accredited Nurse Practitioner program. Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN). Current DEA registration required A minimum of one (1) year of experience working as an APP at an AASM Accredited Sleep Disorders Center or under the supervision of a board-certified sleep medicine Physician. Required Qualifications (PA) Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP. Certification by the National Commission on Certification for Physician Assistants (NCCPA). Current license to practice as a Physician Assistant in the State of Michigan. State of Michigan Controlled Substance License Required Current DEA registration required A minimum of one (1) year of experience working as an APP at an AASM Accredited Sleep Disorders Center or under the supervision of a board-certified sleep medicine Physician.
