What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:

  • Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
  • Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
  • Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
  • Continuous visits and interdisciplinary assessment of the patient.
  • Deliver quality care and completes clear, concise, and accurate clinical documentation.
  • Provide patient, caregiver and family counseling and education as indicated in the plan of care.
  • Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN Case Manager with Care Initiatives Hospice:

  • Active Iowa RN license.
  • Active CPR certification.
  • Hospice or palliative care experience.
  • Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
  • Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
  • Home access to the internet.
  • Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
