School LPN Nurse Needed in the Bronx
2 days per week, 9am-5pm!

Scope of Services
Consultant will coordinate and oversee the health care services across the 2 sites.
This includes:
-Completing all screenings including vision, height, dental, weight and lead. Dental and vision will be monitored and charted if completed prior by the physician. Weight will be monitored and recorded if completed prior and nurse will do follow up after 3 months. This also includes managing all health and nutrition components of the programs in accordance with federal and state regulations, implementing the medication policy including administration of medication, measuring blood pressure, blood sugar and monitor records to ensure compliance with regulatory standards, and function as part of a cohesive team that develops and monitors program operations.

Conducting Physical Assessments:
-Assist all two center base sites with health related information for each child and review all medical information and charts.
-Document visits at all sites for data collection in accordance with all guidelines for health- related child outcomes.
-Review child physicals upon enrollment and on a regular basis thereafter to ensure appropriate updates and sustaining of ongoing physical updates.
-Ensure all physicals have been completed within the last year.
-Ensure the child is up to date with vaccinations.
-Provide guidance for the completion of Child Health Care Plans and monitor every six months for update compliance, renewals, or changes in medication and to update
doctor/parent permission to continue administering.
-Review of any medications that are related to allergies or specific medical care instructions and note this information in the children's file.

-Complies with medical, nursing, and educational ethics.
-Exhibit strong organizational and administrative skills to implement and promote the health program.
-Case management, health intervention and interdisciplinary collaborating skills.
-Awareness of community resources and referral information.
-Advanced skills in interpersonal communication and health counseling.
-Implement advanced nursing protocols on pediatrics and adolescent medicine .

LICENSE/CERTIFICATION:
-License and current registration as an LPN in the State of New York
-Bilingual Spanish Required

Licensed Practical Nurse LPN (P/T 16 hours) Great pay, Weekly Salary. Position Summary: Reporting to the Registered Nurse will provide for medication dispensing and nursing assistance in the diagnosis, and continuous primary medical care to participants. Will act and function as the HIV Coordinator of services for the clinic. Direct Supervisor: Medical Director Responsibilities: Assists the Registered Nurse in the performance of the Registered Nurse's duties. In the absence of the Registered Nurse the LPN will perform all Registered Nurse Responsibilities to assure the continued operation of the clinic. Verifies the identification and observes the behavior of patients to whom medication or other medical services are provided. Will act as the HIV Coordinator for the clinic. This includes the responsibility for tracking the HIV patients for medical care. Must adhere to all procedures referable to accounting for medication dispensed to patients and reporting any deviations immediately to the Registered Nurse. Maintains the medical charts, the medical unit appointment book and daily visit logbook. Performs the appropriate independent nursing follow-up care of patients. Assists in the identification and recall of patients for follow-up medical and nursing care. This includes the monitoring of urine toxicology. Assists in the performance of admission and physical examinations. Provides the physician or physician assistant with urine toxicology and other laboratory data as they become available. Attends on a punctual basis clinic meeting. Performs any and all tasks necessary to improve the quality of care, to improve the productivity of medical staff, and to improve participants satisfaction with the medical/nursing care they receive.
