Nurse Practitioner(NP) Valiant Care Initiative

$130,630 - $200,734 / year
Mission Statement Michigan Medicine improves the health of patients, populations and communities through excellence in education, patient care, community service, research and technology development, and through leadership activities in Michigan, nationally and internationally. Our mission is guided by our Strategic Principles and has three critical components; patient care, education and research that together enhance our contribution to society. Job Summary Opportunity Overview Michigan Medicine is pioneering a new model of health care delivery through the Valiant Care Initiative - a first-of-its-kind, virtual-first, AI-enabled practice housed within the Office of Innovation. This initiative will be launched first in Primary Care to remake the foundation of our health care system and forge future models of care delivery to scale across subspecialties and care settings. It sits intentionally outside traditional departmental structures to enable rapid-cycle testing, bold experimentation, and continuous learning. We are seeking a visionary Nurse Practitioner (NP) to help shape what the future of primary care - and health care - looks like at one of the nation's leading academic medical centers. In this role, you will serve as both a clinician and an innovator - delivering high-quality, relationship-centered primary care while simultaneously building, testing, and refining the digital and AI-enabled tools that will define the next generation of care delivery at Michigan Medicine. About The Valiant Care Initiative The Valiant Care Initiative represents Michigan Medicine's commitment to reimagining health care delivery from the ground up, starting with Primary Care and eventually scaling to additional Specialties. Rather than retrofitting innovation into an established practice model, this initiative begins with a blank canvas - building a learning model designed for adaptation and iteration. Key features of the initiative include: Virtual-First Care Delivery: Leveraging telehealth, asynchronous care, and digital communication tools as the primary care modality, with in-person touchpoints whenever appropriate. AI-Enabled Clinical Practice: Integrating artificial intelligence tools across the care continuum - from triage and care gap identification to clinical decision support and administrative efficiency. Asynchronous Care Models: Exploring and scaling patient-provider interactions that occur outside the traditional synchronous visit, including secure messaging, remote monitoring, and AI-assisted workflows. Rapid Cycle Innovation: Operating within the Office of Innovation and working closely with embedded "forward" engineers as innovation partners to allow nimble testing of new approaches, with the explicit goal that lessons learned will inform system-wide primary care transformation at Michigan Medicine. Interdisciplinary Team: Multi-disciplinary partnership including physicians, nurses, pharmacists, therapists, clerical staff, and others such as digital health specialists in a collaborative, flat-hierarchy model that emphasizes shared accountability, well-being, innovation, and individual role optimization. The Valiant Care Model: Primary Care Our Goal Is Not To Create An Exclusive Concierge Model Or a Transactional Occupational Health Practice; It Is To Build a Longitudinal, High Tech, Relationship-driven Model Of Primary Care. Like Established Primary Care Services, It Will Provide Comprehensive Primary Care: Preventive, episodic, and chronic disease management delivered with continuity and care coordination. Specialist Access: Seamless coordination with specialty services across Michigan Medicine's integrated health system. Proactive Medicine: A culture of prevention and health promotion, not just reactive illness management. Belonging and Engagement: A practice environment where patients are known, valued, and engaged as partners in their health. How the Valiant Care Model differs from traditional primary care services: Virtual-first: Clinicians will be focused on establishing and providing services that initially start with telehealth and asynchronous options that free providers from the typical constraints of brick-and-mortar medical practices that exist today, except when medically indicated. AI-augmented tools: New and innovative approaches that leverage commercially-available and Michigan Medicine-developed AI algorithms to enhance care experiences will be a part of Valiant Care. Examples include access to new generative AI tools for intake and patient education as well as to assist clinicians with record review. A multi-disciplinary team-based care environment in which every member of the team is committed to an innovative mindset and to forging a stellar patient-provider experience. Responsibilities* Clinical Practice Provide comprehensive primary care to adult and pediatric patients - including preventive, episodic, and chronic disease management via virtual-first and in-person modalities. Serve as primary care provider of record for a defined panel of patients, emphasizing continuity, trust, and longitudinal relationships. Actively collaborate within an interdisciplinary team. Leverage and evaluate virtual care, AI-assisted, and asynchronous care tools in day-to-day clinical practice. Innovation Champion the use of emerging digital health tools, providing structured feedback on their clinical utility, usability, and impact on patient outcomes. Partner with the Office of Innovation to design, test, evaluate and iterate on new care delivery workflows - functioning as both a participant and an architect of change. Serve as a health system champion for primary care innovation, contributing to the translation of learnings from the Valiant Care Initiative into broader Michigan Medicine practice. Engage with clinical informatics, data analytics, and digital health teams to evaluate and optimize AI-enabled tools in practice. Contribute to educational and scholarly activities related to innovation, digital health, and value-based primary care delivery.
