Now Hiring: Licensed Vocational Nurse (LVN)

Responsibilities:

  • Provide direct nursing care to residents, including medication administration, wound care, and monitoring of vital signs.
  • Conduct comprehensive assessments of residents' medical and social needs and develop individualized care plans in collaboration with interdisciplinary team members.
  • Coordinate access to medical care, mental health services, substance abuse treatment, and other supportive services to address residents' social determinants of health.
  • Provide counseling, crisis intervention, and support to residents and their families to address psychosocial concerns and promote coping skills and resilience.
  • Advocate for residents' rights and access to resources, including housing, benefits, and community support services.
  • Collaborate with community partners and organizations to facilitate access to resources and services for residents transitioning to permanent housing.
  • Maintain accurate and up-to-date documentation of care plans, interventions, and resident progress in electronic health records (EHR) systems.

Requirements for LVN (Licensed Vocational Nurse):

  • Current, valid license or certification in California as an LVN
  • Compassionate and empathetic attitude towards individuals from diverse backgrounds
  • Previous experience in a similar role, preferably in a healthcare or social services setting
