As an Acuity Adaptable Unit (AAU) Level of Care is flexible and adapts to patient needs. This ranges from Non-Monitored to PCU level of care. As a Registered Nurse on 5South, you are responsible and accountable for the application of the nursing process and the delivery of patient care for the specialty patient population. Job Summary Provides quality patient care to critically ill patients, through the nursing process of Assessment, Planning, Intervention, and Evaluation. Serves as a patient advocate at all times. Supervises the nursing care provided to each patient by other caregivers. Develops the plan for nursing care as a result of coordination with the patient, family and staff of other disciplines as necessary. Applies critical thinking in order to promote positive clinical outcomes. Ensures patient education. Participates in discharge planning. Essential Functions Maintains the standard of nursing care and implements the policies and procedures of the hospital and the nursing department. Assesses the patient on admission within the required time frame. Makes referral to other disciplines based on the assessment, such as rehabilitation and nutritional services. Develops a nursing care plan for assigned patient on admission and updates the plan of care as needed. Ensures the plan is discussed and developed as a result of the coordination of patient and family and the interdisciplinary team. Able to release, remove, and reapply restraints to patient. Ensures patient is in proper position and alignment. Documents patient admission assessment and reassessments, patient care plans, and other pertinent information completely in the patient’s electronic medical record. Demonstrates correct use of Biomedical equipment such as defibrillator, Suction machine, IV and feeding pumps, PCA pump, blood glucose monitoring machine. Demonstrates safe and effective delivery of patient care such as completing team/patient assignments on time, making effective use of available time, showing initiative and teamwork. Obtains consents for procedures and blood transfusions, ensuring patient has necessary information to make an informed decision. Receives physician’s orders, ensures transcription is accurate, and order entry is accurate. Follows through to ensure order is carried out. Recognizes and responds appropriately to patient change in condition such as Cardiac rhythm, respiratory distress, GI bleed, pulmonary emboli/ edema, diabetic Ketoacidosis, adverse drug reaction and Blood transfusion reaction, etc. Institutes emergency action, notifies physician, obtains orders, etc. Completes and documents patient admission assessment and reassessments as required according to policy. Reports pertinent observations and reaction regarding patients to the appropriate person (i.e., physician, supervisor, charge nurse) and records these observations accurately. Administers medications with verbalized knowledge of the action, side effects, correct dosages and route according ICU policies and procedures. Demonstrates ability to recognize and Institute arrhythmia protocol for the following Cardiac rhythms V-tach, V-fib, PVC’s, asystole, 1st, 2nd and 3rd degree heart block, atrial fibrillation, and atrial flutter. Directs, supervises, and evaluates nursing care provided to patients. Administers medications utilizing the seven “rights” of medication administration: right patient, route, time, medication, dose, reason and documentation. Assist physicians with special tests and procedures such as thoracentesis, paracentesis, arterial line and bronchoscopy. Participates in economical utilization of supplies and ensures that equipment is maintained in a clean and safe manner. Participates in orientation of new personnel and performance evaluations of nursing staff. Assists team members in giving care to patients or administers direct care when professional nursing skills and judgment are needed. Coordinates nursing care of patients when scheduled for therapy or procedures by other departments. Assists with or institutes emergency measures for sudden adverse developments in patients such as cardiac arrest, respiratory arrest, or sepsis. Participates in the maintenance of a clean and safe environment. Is knowledgeable about the emotional and rehabilitative aspects of care and is capable of applying appropriate therapeutic interventions. Provides assessment utilizing current ACLS certification and protocols renders lifesaving first aid and makes appropriate referral to the nearest organization that is capable of providing needed services. Demonstrates ability to manage various types of tubes and other devices such as Foley /straight catheter, feeding tubes, chest tubes, JP drains, Hemovacs. T-tubes, wound drains, ostomy care, suprapubic catheter, continuous bladder irrigation. Demonstrates correct set up and performance monitoring of hemodynamic systems such as Swan-ganz catheter, arterial catheter, and cardiac output. Demonstrates accurate knowledge of critical Lab values and their normal ranges. Reports and documents appropriately. Demonstrates ability to verify correct lead telemetry placement. Validates telemetry strips every 4 hours and PRN. Documents appropriately. Participates in assessing and meeting patient and family education needs. Provides complete patient report to oncoming RN, ensures documentation is complete, physician orders are signed off, and patient rooms are in order. Communicates pertinent information to RN, physician, and patients as needed. Adheres to dress code, appearance is neat and clean and wears appropriate identification while on duty. Completes annual health, safety, and education requirements. Maintains professional growth and development. Maintains confidentiality of all patient and/or employee information to assure patient and/or employee rights are protected. Certified Nursing Assistant (CNA) QUALIFICATIONS • Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant. • A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled in a nursing assistant certification class. • Must possess a sincere desire to serve the ill, aged and infirm. • Ability to read, write and speak the English language. Certified Nursing Assistant (CNA) GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. • Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. • Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Certified Nursing Assistant (CNA) CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30° for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
