
Street Medicine Physician Assistant / Nurse Practitioner
Rafi Health is seeking a full-time Physician Assistant (PA) or Nurse Practitioner (NP) with prior street medicine experience to deliver care directly to individuals experiencing homelessness across Los Angeles. This is a field-based role: you'll meet patients where they live — in encampments, shelters, and other outdoor settings — bringing high-quality, compassionate, trauma-informed care into the community.
Working collaboratively, you'll provide primary, acute, preventive, and chronic care while helping patients navigate their ongoing medical and social needs. This role is ideal for a clinician who is passionate about health equity, thrives outside the traditional clinic, and is committed to serving some of LA's most underserved communities.
Responsibilities
Provide comprehensive primary, acute, preventive, and chronic care to individuals experiencing homelessness and other underserved populations.
Perform evaluations, physical exams, diagnosis, treatment, and medication management.
Develop care plans and coordinate referrals, follow-up, and community resources.
Build trusting relationships with patients through compassionate, trauma-informed care.
Document encounters and support continuity of care across settings.
Qualifications
Active, unrestricted California PA or NP license.
Active DEA registration (or eligibility to obtain prior to start).
Prior street medicine, mobile clinic, or field-based experience caring for unhoused or high-need populations.
Strong clinical judgment and the ability to work both independently and collaboratively in dynamic field environments.
Valid California driver's license and reliable transportation.
Preferred Qualifications
Experience with substance use disorder treatment, including buprenorphine/MOUD.
Familiarity with LA-area homeless services, community resources, and referral networks.
Bilingual (Spanish/English) or other second language.
Work Environment
This is a field-based, outdoor role. You'll deliver care in dynamic and sometimes unpredictable settings, be assigned to locations throughout Los Angeles, and adapt to varying weather and field conditions. Comfort working outside a traditional clinical setting is essential.
Compensation & Benefits
Salary range: 140,000–170,000, commensurate with experience
