Job Description

REGISTERED NURSE – HOME HEALTH

Location:

  • City of Chicago, west side area which includes Logan Square and surrounding communities


Schedule:

  • Monday through Friday, 8a-5p daytime hours.
  • Rotating weekend coverage every third weekend.
  • Participation in the on-call rotation, including 3–4 weeknight on-call shifts per month and weekend on-call coverage approximately every 1–2 months.
  • Holiday rotation includes one winter holiday and one summer holiday annually.

This position is eligible for up to a $3,000 sign-on bonus. You must be actively working in this same position at the time of payout. Current internal team members are not eligible at this time.

Licensure:

  • Nurse, Registered (RN)

  • A valid driver’s license issued by the Division of Motor Vehicles.

Education/Experience Required:

  • Graduate from an accredited school of nursing

  • Earn 10 continuing education credits annually

  • One year of medical surgical experience

  • Has as demonstrated proficiency for the requirements for RN role

Knowledge, Skills & Abilities Required:

  • Good communication and interpersonal skills

  • Good documentation skills.

  • Computer skills

  • Good time management and organizational skills

  • Familiarity with patient care equipment

  • Current Illinois RN license

  • Current CPR


Physical Requirements and Working Conditions:

  • Ability to work weekends, holidays, and different shifts in order to accommodate staffing

  • Ability to take “on call” assignments.

  • Ability to handle multiple tasks and complete work within short timeframes.

  • Ability to transport objects, patients, and equipment

  • Ability to respond quickly to patient needs and work at fast pace

  • Work environment includes probability of exposure to adverse, hazardous, or unpleasant conditions while caring for the sick

  • Ability to lift up to 100 pounds occasionally and/or in excess of 50 pounds frequently and/or in excess of 20 pounds constantly

  • Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads.

  • Exposed to widely varying social and environmental conditions in patient’s neighborhoods and homes.

  • Must be willing to work occasional overtime if needed. Required to make visits to private residences which may not be accessible to people with disabilities.

  • Must be able to assist with ambulation including turning and transferring patients up to 200 pounds

  • If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.

Positions are available in assisted living, skilled nursing and memory care. This position to ensure that quality care is delivered through assessment and monitoring of staff performance, physician contact, care planning and daily resident assessment. This position also provides direct care to residents. All this is done according to nursing standards of practice, standard procedures, and established State and Federal regulations for skilled nursing facilities. In this role, the Registered Nurse will: Provide routine daily direction and monitor the work of the LPNs and nursing assistants on the unit, assuring a high standard of nursing care and satisfactory completion of tasks. Assist in the orientation of new nursing personnel and reinforce training standards to maintain a high quality of care. Assist in motivating LPNs and CNAs assigned to the nursing unit. Provide explanation of nursing procedures and assistance to staff to assure ongoing understanding and training. Communicate on a regular basis with the Director of Nursing regarding assessments, observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc. Observe and evaluate residents' health, mental and emotional conditions and communicate significant changes to appropriate staff. When indicated is responsible for notifying the physician and family members according to prescribed procedures. The ideal candidate will have: Graduate of an approved school of nursing. Current license as a Registered Nurse in the State. One year nursing experience necessary. Devotion to standards and principles of professional nursing. Skill in assessing problems, planning and evaluating solutions in relationship to routine supervision of LPNs and nursing assistant and resident needs. Understanding, patience and tact in dealing with people. Must be tactful and possess excellent communication skills (written and verbal, English). Genuine compassion for senior adults and the disabled. Requires good human relation skills and the ability to monitor and direct the activities of others. Availability when necessary to work beyond normal working hours and in other related positions on a temporary basis; as well as availability on weekends and holidays, or on days and shifts other than those for which hired. #RN Compensation Pay Range: $36.90 - $51.94 per hour Reasonable Pay Estimate A reasonable estimate of the pay range for this position is $36.90 - $51.94 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
