About the Role:

As a Regional MDS Coordinator with Aspire Senior Living, you will be responsible for coordinating and maintaining accurate and timely Minimum Data Set (MDS) assessments for residents in our skilled nursing facilities. This includes conducting resident interviews, reviewing medical records, and providing education and support to our care team. Your expertise and attention to detail will ensure that our residents receive the highest level of care possible.

Responsibilities:

  • Coordinate and conduct accurate and timely MDS assessments for residents in our skilled nursing facilities

  • Review medical records and conduct resident interviews to gather necessary information

  • Provide education and support to our care team to ensure compliance with MDS regulations

  • Identify and report concerns or issues related to resident care and quality of life

  • Collaborate with other departments to ensure seamless transitions and comprehensive care

  • Maintain accurate and up-to-date records and reports

Requirements:

  • Bachelor's degree in Nursing or related field

  • Experience in a skilled nursing setting, preferably in MDS coordination

  • Current licensure as a Registered Nurse in the state of Missouri

  • Strong knowledge of MDS regulations, guidelines, and best practices

  • Excellent communication, interpersonal, and organizational skills
