Job Summary
Premier Rehab & Healthcare at Burlington is seeking an experienced and compassionate Unit Manager – Long-Term Care (LTS) to join our nursing leadership team.
As the LTC Unit Manager, you will oversee the daily operations of the Long-Term Rehabilitation unit while ensuring the delivery of exceptional, resident-centered care. This leadership position is responsible for supervising nursing staff, coordinating interdisciplinary services, monitoring clinical outcomes, and maintaining compliance with state and federal regulations.
The ideal candidate will have strong clinical and leadership experience, a background in rehabilitation or long-term care, and a passion for supporting both residents and staff.
Responsibilities
Oversee the daily operations of the Long-Term Rehabilitation unit.
Supervise and support nursing staff to ensure the delivery of high-quality resident care.
Collaborate with physicians, therapists, social workers, and other interdisciplinary team members.
Coordinate and implement individualized resident care plans.
Monitor resident assessments, treatments, and clinical outcomes.
Ensure care is delivered in accordance with facility protocols and safe nursing practices.
Participate in resident admissions, discharges, and care transitions.
Ensure compliance with state, federal, and facility regulations.
Provide leadership, guidance, mentoring, and support to nursing staff.
Participate in quality improvement initiatives and performance monitoring.
Communicate effectively with residents, families, and members of the care team.
Foster a collaborative, team-oriented, and resident-focused environment.
Support staff development and promote high standards of clinical care.
Qualifications
Current, active, and unrestricted Vermont Registered Nurse (RN) license.
Previous leadership, supervisory, or unit management experience preferred.
Experience in short-term rehabilitation, skilled nursing, subacute rehabilitation, or long-term care preferred.
Strong clinical assessment and critical-thinking skills.
Knowledge of state and federal long-term care regulations.
Excellent communication and interpersonal skills.
Ability to manage multiple priorities in a fast-paced healthcare environment.
Strong leadership and organizational skills.
