Full Time Registered Nurse Case Manager, Level 2 Anticipated Territory: Sonoma and surrounding areas   What you’ll do as a Registered Nurse (RN), Level 2 Home Health Case Manager:   Utilize your outstanding clinical skills as you create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.   In this leadership role, you will have oversight of 2 Nurses who are responsible for executing your care plan. You will be responsible for conducting comprehensive patient assessments such as Start of Cares (SOCs), Evaluations, Resumption of Care and discharge planning.   You will accomplish this with oversight of the assistance and teaching of patients’ and their family/caregivers with wound care, blood draws, tube feedings, Tracheostomy care, foley catheters, Congestive Heart Failure (CHF) management and medication education in the patients home. You will also potentially:
  • Perform a full assessment of the patient & family upon initial visit.
  • Use assessment data to develop an advanced plan of care including rehabilitation.
  • Work with patients and family one on one to achieve improvement and/or stability.
  • Case manage patients care episode with interdisciplinary clinical team.
  • Prepare clinical notes, orders and charting in a compliant time frame.
  • Collaborate with physicians and agency staff to deliver the best care possible
  • Have oversight of 2 Nurses who are responsible for executing your care plan.

To be a success a Registered Nurse (RN) Home Health Case Manager, Level 2:

  • One Year of Home Health Case Management experience is perferred
  • Minimum of one year of Nursing Experience required; two+ years preferred.
  • Experience supervising, mentoring Nurses is highly preferred
  • Current and unrestricted CA RN license
  • Current BLS/CPR Certification
  • Valid CA Driver's License
  • High level of clinical knowledge in home health care is key
  • Experience with high acuity patients is a plus
  • Experience working with an interdisciplinary team is required

As Home Health Field Nurse you will: Providing hands-on patient care including, but not limited to (must be within RN license scope): PICC lines, Wound Vacs & Care ,Kangaroo & feeding Pumps, Management of NG, G, and J tubes, Staple & suture removal ,Drains such as, JP, Biliary, Nephrostomy, ,Foley/suprapubic catheters (insertion, removal, and troubleshooting), PleurX drainage system ,Trach care, Blood draws, Ostomy management ,SQ and IM injections, IV antibiotic/TPN administration and Compression wraps. Provides direct patient care as defined in the state Nurse Practice Act Implements a plan of care initiated and directed by the Case Management Registered Nurse Provides accurate and timely documentation within 24-48 hours consistent with the plan of care Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care Assists the registered nurse in performing skilled needs procedures and duties, which include preparing equipment and materials for treatments and assisting the patient in learning appropriate self-care techniques Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the Case Management RN or other disciplines Uses equipment and supplies effectively and efficiently Participates in personal and professional growth and development Performs other duties as assigned by the Case Management Registered Nurse Retrieves communication on assigned patients. Uses information received as a basis for establishing priorities of care Gives a timely and pertinent report to the Case Manager Records pertinent and concise information that will reflect the patients' needs, problems, capabilities and limitations, as well as the patients' response to nursing interventions Entries will accurately record any incident that has a bearing on the patient. Reports pertinent information timely to the Case Management Registered Nurse and to Provider Attends weekly Case Conferences as required to coordinate and communicate information regarding patient care and treatment Ensures patient privacy in maintaining medical records and when providing care. Maintains professionalism and seeks opportunities to enhance knowledge of clinical skills Maintains productivity standards as directed by the Case Management Registered Nurse . Completes and reviews Start of Care assessments, documentation, and plans of care with Case Manager, as needed. Participates in on-call duties as required by weekend rotation To be a Success, you must: One (1) year of recent nursing experience Experience requirement may be waived for internal candidates and/or new grad posting. One year of home health care experience is preferred. Job Requirements, Prerequisites and Essential Functions: Pay Range: $62.78 - $78.47 - $94.16 Essential Functions and Responsibilities: Individual Triages patients’ physical symptoms/arrhythmias appropriately and consults with cardiologists per protocol. Coordinates patient care for Testing Services which includes bubble studies and/or definity. May perform cardiac stress test(s) as needed and / or coordinate care with the Exercise Physiologist as needed. Works with assigned Cardiologist(s) to manage in-basket, assess for symptoms and provide appropriate patient follow-up. Provides pre, post-op education for Cardiology procedures. Educates patients about their cardiovascular condition and explains treatment options as directed by the cardiologist. Teaches patients what they can do to prevent the worsening of cardiovascular conditions, the importance of self-monitoring and provide appropriate resources. Participates in Transition of Care Management to ensure discharge medications and instructions are understood by patient and/or family members. Updates medication and problem list in the Electronic Health Record and ensures follow-up with cardiologists. Reviews daily lab/test results, physician tasks and clinical worklist(s) as assigned. Assist the Nurse Supervisor and/or Director on matters relating to nursing practices and participates in development of policies and protocols for patient care.
