RESPONSIBILITIES With dignity, compassion, and care, your duties include, but not limited to:

This position is ideal for a caregiver who genuinely enjoys building relationships with clients and wants a stable 36 hour weekly schedule with one consistent client . We are looking for someone who brings positive energy into every shift. The right caregiver will be conversational, attentive, observant, patient, compassionate, safety focused, and comfortable remaining actively engaged throughout a full 12 hour day. This is not an assignment for someone who simply wants to complete a few tasks and remain disengaged for the rest of the shift. We need a caregiver who takes appropriate initiative, enjoys companionship, encourages the client’s normal daily routine, and understands that excellent caregiving involves both hands on support and meaningful engagement . REQUIRED WEEKLY SCHEDULE Wednesday from 7:00 AM to 7:00 PM Friday from 7:00 AM to 7:00 PM Sunday from 7:00 AM to 7:00 PM 36 scheduled hours every week Immediate start The selected caregiver must be able to consistently work ALL THREE DAYS every week . This assignment is being staffed specifically for continuity of care. We are not looking to divide these three days among multiple applicants at this time. ABOUT THE ASSIGNMENT This is a consistent one on one private duty assignment for a client who benefits from an attentive, energetic, professional caregiver who understands safety, companionship, personal care, and maintaining a positive daily routine. The caregiver must be comfortable providing close supervision when needed, particularly during mobility and bathroom related activities, and must understand the importance of fall prevention, observation, and timely communication of changes or concerns . The successful caregiver will encourage independence whenever it can be done safely while providing appropriate assistance according to the client’s established care plan. JOB RESPONSIBILITIES The selected caregiver must be comfortable performing the following duties as authorized by the client’s care plan and Helping Hearts policy: Companionship and Active Engagement Remain genuinely engaged with the client throughout the shift through conversation, appropriate activities, encouragement, social interaction, and participation in the client’s normal daily routine. Engagement means interacting with the client rather than simply being physically present in the room. Personal Care Assistance Provide respectful assistance with activities of daily living while maintaining the client’s privacy, dignity, comfort, and independence. This may include bathing, showering, grooming, oral hygiene, dressing, toileting, hygiene assistance, and incontinence care when required. Bathing and Shower Assistance Help prepare for and safely complete bathing or showering. Remain attentive to balance, weakness, dizziness, slipping hazards, and fall prevention throughout personal care activities. Dressing and Grooming Assist with clothing, shoes, fasteners, hair care, oral hygiene, and other routine grooming needs while encouraging the client to safely participate as much as possible. Toileting and Bathroom Assistance Provide safe assistance to and from the bathroom, help with clothing management and hygiene, and remain available when the client requires supervision or assistance. Bathroom safety is an important part of this assignment. Mobility and Ambulation Assistance Provide the level of walking or mobility assistance identified in the client’s care plan. This may include standby assistance, walking beside the client, and assisting with approved mobility equipment. Transfer Assistance Assist with approved transfers within the caregiver’s training and according to the client’s care plan. Proper body mechanics and safety procedures must always be followed. Caregivers must never attempt a transfer that cannot be completed safely. Fall Prevention and Safety Monitoring Maintain awareness of the client’s environment, walking surfaces, mobility, balance, footwear, and potential hazards. Keep pathways clear and promptly report falls, near falls, sudden weakness, dizziness, or meaningful changes in mobility. Meal and Snack Preparation Prepare appropriate meals and snacks according to the client’s preferences and care instructions. Assist with meal setup, encourage appropriate intake, clean the kitchen area after use, and observe for significant changes in appetite. Hydration Support Offer and encourage appropriate fluids throughout the shift and report meaningful changes in the client’s normal fluid intake. Medication Reminders Provide medication reminders only as permitted by Helping Hearts policy and the client’s care plan. Caregivers do not independently change medication dosages, make medication decisions, or perform duties outside their authorized role. Daily Routine Support Help maintain a consistent and positive daily routine involving personal care, meals, hydration, activity, companionship, movement, and appropriate rest. Activities and Socialization Encourage activities appropriate to the client’s interests and abilities. This may include conversation, games, reading, music, television programs, appropriate movement, or participation in approved activities. Light Housekeeping Maintain a clean and safe client environment. Duties may include cleaning up after meals, washing dishes, wiping counters, straightening the client’s living area, keeping the bathroom tidy, removing trash, making the bed, and maintaining clear walking pathways. Client Laundry and Linens Complete client related laundry when needed, including washing, drying, folding, putting away clothing and linens, and changing bed linens as appropriate. Environmental Safety Observe for spills, clutter, blocked pathways, unsafe furniture placement, misplaced equipment, or other hazards that could increase the client’s risk of injury. Observation of Changes in Condition Remain highly observant throughout the shift. Report significant changes involving mobility, balance, weakness, dizziness, alertness, behavior, appetite, hydration, bathroom habits, communication, energy level, or overall condition. Caregivers are not expected to diagnose medical conditions. They are expected to observe, document, and promptly report meaningful changes from the client’s normal baseline . Incident and Safety Reporting Immediately notify Helping Hearts Operations of significant concerns such as falls, near falls, sudden weakness, significant changes in condition, refusal of important care, safety concerns, or other reportable incidents. WellSky Documentation Complete all required documentation accurately and timely through WellSky Personal Care . Documentation must clearly reflect care provided, observations, client status, activities completed, and relevant concerns. Communication With Operations Maintain clear, timely, and professional communication with Helping Hearts Operations. When an update is requested, caregivers must provide a meaningful written response rather than a one word answer. Professional Boundaries Maintain appropriate boundaries with the client and family. Scheduling, pay, additional hours, time off, and employment related matters must be handled through Helping Hearts Operations. Shift Handoff Provide an appropriate handoff to an incoming caregiver when applicable, including relevant observations, safety concerns, changes in routine, and unfinished care needs. Full Shift Commitment Remain for the complete scheduled 7:00 AM to 7:00 PM shift unless released by Helping Hearts Operations. Emergency Response Remain calm, protect the client from additional harm, contact emergency services when appropriate, follow Helping Hearts emergency procedures, and notify Operations promptly. REQUIRED SKILLS The strongest candidate will have demonstrated experience with personal care, bathing assistance, toileting support, safe mobility assistance, fall prevention, client supervision, meal preparation, hydration encouragement, companionship, recognizing changes from baseline, professional documentation, and communicating concerns appropriately. We also need someone with strong situational awareness and judgment . You should know when to provide assistance, when to encourage independence, when something does not appear normal, and when it is time to immediately contact Operations. THE PERSONALITY WE NEED Clinical skills and caregiving experience matter, but personality matters for this assignment too. We are looking for someone who is upbeat without being overwhelming, confident without being controlling, compassionate while maintaining professional boundaries, observant without being intrusive, patient, conversational, encouraging, and naturally engaging . The ideal caregiver does not need to be reminded repeatedly to interact with the client or find appropriate ways to keep the day positive and productive. We need someone who walks into the assignment ready to make a positive difference. RELIABILITY IS ESSENTIAL Continuity of care is extremely important. The client, family, and agency need to be able to depend on the selected caregiver every: Wednesday from 7:00 AM to 7:00 PM Friday from 7:00 AM to 7:00 PM Sunday from 7:00 AM to 7:00 PM Applicants with repeated availability conflicts, frequent call outs, ongoing schedule change requests, or an inability to consistently arrive for a 7:00 AM start will not be the right fit for this assignment. PROFESSIONAL APPEARANCE Helping Hearts caregivers represent our agency every time they enter a client’s environment. Navy scrubs are required. Professional closed toe footwear is required. Crocs are not permitted. Caregivers are expected to maintain a clean, neat, and professional appearance throughout the assignment. QUALIFICATIONS Previous professional caregiving experience is strongly preferred, particularly experience providing one on one care to older adults or clients requiring mobility and fall prevention support. Experience as a Caregiver, CNA, Home Health Aide, Personal Care Assistant, Direct Support Professional, or similar direct care professional is preferred.
