The afterhours LVN supports our family’s needs after hours.  After hours is qualified as after 5PM and weekends. The professional nurse is accountable to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. Requirements

Patient Care

 The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process.

HOSPICE FIELD NURSE RN/LVN

Full job description If you would prefer to work with an agency that honors and cares for its clinicians, with the strong communications and teamwork this is your place . Anchor Health is seeking a Registered Nurse and Licensed Vocational Nurse to serve in the San Francisco County . The Registered Nurse will plan, organize and direct hospice care. They will also build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Position Responsibilities: - Coordinating the care of the patients assigned to you. - Making regular visits to patients' homes to assess patients' physical, emotional, spiritual, and social needs. - Working collaboratively with the hospice physician and your supporting LVNs to carry out appropriate interventions. - Helping the agency to continuously increase the quality of care. Position Requirements: - Licensed as a Registered Nurse/RN - Valid Driver's License and reliable vehicle with current insurance. - Minimum of two years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. - CPR Certified.
