Coordinatescare for patients throughout the continuum of care from diagnosis to therelease from care. Uses amultidisciplinary approach and acts as a liaison between the patient, staff andreferring physician offices as well as ancillary departments of the hospitalsystem. Identifies learning needs and content for educational programs.Develops, presents, coordinates and evaluates educational activities.Effectively performs in the role of clinician, facilitator, advocate,consultant and/or researcher. The NurseNavigator will guide the patient/family through the complexities of thehealthcare process, working to eliminate barriers that might otherwiseadversely impact patient care/outcome and will help to coordinate the effortsof the medical team to facilitate continuity of care and enhance patientsatisfaction.
