Licensed Practical Nurse (LPN) Duties and Responsibilities:

  • Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations 

  • Provides and receives nursing reports upon reporting in and ending shift duty hours. 

  • Ensures assigned work area (i.e. nurse station, medication cart, medication room, etc.), resident care rooms and treatment areas are maintained in a clean and sanitary manner.

  • Directs and supervises the day-to-day certified nursing assistant services for assigned unit to assure that care is being rendered in accordance with current federal, state and local standards, guidelines and regulations. 

  • Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care. 

  • Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. 

  • Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record (“MAR”), treatment/care plans, as required. 

  • Prepares and administers medications and performs treatments as ordered by the physician. 

  • Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. 

  • Completes the required forms upon admission, transfer, and/or discharge. 

  • Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. 

  • Makes written and oral reports/recommendations concerning the activities of shift as required. 

  • Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards.

  • All other duties, as assigned.

Licensed Practical Nurse (LPN) Skills and Abilities:

  • Excellent written and verbal communication skills in English. 

  • Ability to multi-task and remain calm under stress. 

  • Analytical and problem-solving skills. 

  • Understands and follows infection control standards, policies and procedures.

  • Must be able to adapt to a flexible work schedule and occasional overnight travel.

  • Demonstrate basic computer knowledge and ability with an aptitude to learn company applications. 

Licensed Practical Nurse (LPN) Education and Experience:

  • Current CPR Certification required.

  • Possess a current license to practice in the State as a Licensed Practical Nurse. 

  • 1 year of experience preferred.

  • Long-Term care experience preferred.

  • Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, all with or without the aid of mechanical devices is required.

The MDS / Care Plan Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. Assists disciplines in formulating and revising care plans. Ensures that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. Evaluates resident care plans for comprehensiveness and individuality. Assesses the achievement or lack of achievement of desired outcomes. Ensures that resident’s care plan is reassessed and revised appropriately. Responsible for all level of care changes within the facility. Notifies all departments when a level of care change has been made. Generates appropriate forms to complete level of acuity and changes.
