Summary: The Registered Nurse (RN) provides direct nursing care to residents and supervises nursing assistants to ensure high-quality care in compliance with federal, state, and local regulations. Key responsibilities include planning and overseeing daily nursing activities, administering medications, completing required documentation, and maintaining patient confidentiality. The RN also performs professional nursing services such as catheterization, wound care, and IV care, while fostering a warm and collaborative environment. Additionally, the role involves coordinating with physicians, ensuring safety protocols are followed, and maintaining effective communication with residents, families, and staff to meet residents' needs. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit and shift. Receive/give the nursing report upon reporting in and ending shift duty hours. Make rounds with physicians as necessary. Assist in all aspects of a resident’s daily life, including activities of daily living, meal service, and social engagement. Appropriately communicate with clinical leadership when a resident’s needs change. Know how to approach and care for residents with dementia. Qualifications: Registered Nurse (RN) with a current unencumbered state license. Must be knowledgeable of nursing & medical practices & procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities. Experience in Long Term Care is preferred. Willing to take additional training or education, which specific states may require. CPR and First Aid certifications are required. Ability to be flexible and adaptable while handling multiple priorities. Possess written and verbal skills for effective communication. Competent in organizational and time management skills. MDS Coordinator

** $10,000 Sign On Bonus! ** (SIGN-ON BONUS SUBJECT TO TERMS AND CONDITIONS, AND MUST MEET MINIMUM HOURS WORKED REQUIREMENT) SUMMARY: The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses and determines the health status and level of care of all new admissions. Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change. Communicates level of care for new residents to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to the facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state, and medical standards. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary, and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conferences. Assists disciplines in formulating and revising care plans. Ensures that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. Evaluates resident care plans for comprehensiveness and individuality. Assesses the achievement or lack of achievement of desired outcomes. Ensures that the resident’s care plan is reassessed and revised appropriately. Responsible for all level of care changes within the facility. Wound Care Nurse

SUMMARY: The Wound Care Nurse can be either a Registered Nurse (RN) or Licensed Practical Nurse (LPN) and is responsible for providing primary skin care to residents under the medical direction and supervisor of the resident’s attending physician, the Director of Nursing, or the Medical Director of the facility, with an emphasis on treatment and therapy of skin disorders. ESSENTIAL DUTIES AND RESPONSIBILITIES: Confer with the Director of Nursing and/or other licensed nursing personnel regarding the dermatologic disorders of residents in the facility Examine the resident and his/her records and charts, and discriminate between normal and abnormal findings, in order to recognize when to refer the resident to a physician for evaluation, supervision, or directions Provide assessment and diagnostic services to residents. Work involves responsibility for supervising the provision of quality nursing care given at the facility. The position is responsible for maintaining the quality and adequacy of nursing practice; coordinating nursing programs; for recognizing and assisting in solving professional, administrative, and supervisory problems. Position exercises considerable judgment in applying professional knowledge in solving nursing problems within established policies and practices.
