About the Role
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) – Extended Hours Hospice Care to join our hospice team in a part-time capacity.
This role provides extended-hours clinical support for hospice patients and families, ensuring they have access to skilled and compassionate care when needs arise outside of traditional business hours. As part of our extended-hours team, you will respond to urgent patient needs, provide direct nursing care, perform assessments, and collaborate with the interdisciplinary team to promote comfort, dignity, and quality of life.
This position travels throughout the entire Blue Ridge Care service area.
Schedule
Part-Time Position
Friday through Sunday schedule
Every other weekend
Extended-hours coverage based on patient and organizational needs
Eligible for applicable shift differentials
How You'll Make a Difference
Respond to emergent calls from patients and families during extended-hours coverage.
Provide urgent visits to address symptom changes, patient needs, or end-of-life concerns.
Deliver skilled nursing care in accordance with patient goals and physician orders.
Assess and evaluate patient conditions and communicate recommendations to the care team.
Administer medications and treatments as prescribed.
Support symptom management and patient comfort.
Educate patients and families on disease progression, symptom management, and care techniques.
Provide emotional support and reassurance during difficult and often unexpected situations.
Communicate significant changes in condition to physicians and obtain necessary orders.
Complete timely and accurate clinical documentation.
Coordinate equipment, supplies, and services needed to support quality care in the home.
Collaborate with the interdisciplinary team to ensure continuity of care.
Assist with crisis visits, death visits, admissions, and other urgent hospice needs as appropriate.
Promote a safe and supportive environment for patients and families.
Participate in ongoing education and quality improvement activities as applicable.
What You'll Bring
For RN Candidates
Current Virginia Registered Nurse (RN) license.
Graduate of an accredited nursing program.
Hospice, home health, ICU, critical care, or related nursing experience preferred.
For LPN Candidates
Current Virginia Licensed Practical Nurse (LPN) license.
Graduate of an accredited practical nursing program.
Hospice, home health, long-term care, or related clinical experience preferred.
For All Candidates
Current CPR certification.
Strong assessment, communication, and clinical decision-making skills.
Knowledge of hospice philosophy and end-of-life care principles.
Knowledge of Medicare and Medicaid hospice regulations preferred.
Proficiency with electronic medical records.
Ability to travel throughout the Blue Ridge Care territory.
Ability to work independently and respond effectively in urgent situations.
