Responsibilities

The Nurse Practitioner focuses on providing general care to patients of all ages, with utmost patience, care, compassion and understanding.� Perform tasks within their scope of practice which may include conducting tests, recording medical histories, providing treatments, assisting physicians and enhancing public awareness on the importance of health, and prescribing medications.�

 

Position Highlights:

  • See patients 18 and older who meet Medicaid requirements and reside in St. Clair County
  • Provide wellness checkups, non-emergency chronic illness care, lab and radiology testing, specialist referrals
  • Work with services such as CMH, specialists, patient guardians, adult foster care staff
  • See a high volume of patients with limited transportation, under/unemployment, mental health diagnosis, and substance use

Compensation/Benefits:

Qualifications

Education and Work Experience

  • MSN with specialization as an ARNP.
  • Current state license as an Advanced Practice Nurse.
  • BLS upon hire and maintain current.

    MAJOR FUNCTIONS: Under general direction, the Nurse Practitioner is responsible under direction of a physician for providing general medical care, preventative care and treatment to patients in one of the agency’s clinics. DUTIES AND RESPONSIBILITIES: Performs routine physical examinations and preventive health measures within prescribed guidelines and instructions of a physician based on assigned program. Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs. Records accurate medical histories, physical findings and formulates plan and prognosis based on patient’s condition. Documentation includes medical evaluations, diagnosis, procedures, treatment, prognosis, referrals, education and consultation. Discusses case with physician and other health professionals and therapeutic team to prepare comprehensive patient care plan. Submits health care plan and goals of individual patients for periodic review and evaluation by physician. Prescribes medications and monitor effectiveness, or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures. Refers patients to physician or to specialized health resources for treatment. Makes patients aware of importance of a healthy lifestyle.
