JOB TITLE: Clinic Nurse Practitioner-TLC/CCC JOB CODE: 1295 JOB HOURS: Various It is expected that all of the duties and responsibilities of this position will be performed in a manner that reflects the Four Key Values of Bothwell Regional Health Center, which are: Community, Purpose, Integrity, and Joy. JOB SUMMARY The Clinic Nurse Practitioner shall have a collaborative agreement with the Physician which grant the APN authority to provide care to patients following protocols that have been jointly agreed upon. The APN provides patient assessment, develops a diagnostic impression, and plans the patient treatment following established protocols. The Clinic Nurse Practitioner reports directly to the Clinic Site Manager who in turn reports directly to the Director of Practice Management. QUALIFICATIONS: Education: a.) Graduate of accredited school of nursing with BS and MS in nursing, including educational preparation as a Nurse Practitioner. Licensure/Certification: a.) Current licensure by Missouri State Board of Nursing. b.) ACLS preferred. c.) BLS required. Work Experience: a.) Past experience as Registered Nurse. b.) Preferably has at least two years as an APN. RESPONSIBILITIES/JOB DUTIES: 1. Assumes responsibility for quality of medical services rendered to patients 2. Consults with Collaborative Physician as needed. 3. Follows all protocols as established with collaborative Physician. 4. Maintains current licensure and required continuing education. 5. Perform other duties as assigned. The establishment of Collaborative Practice Agreement Chapter 257 of the Laws of 1988 of New York State which took effect April 1, 1989, authorizes Nurse Practitioners certified by the State Education Department to use the title "Nurse Practitioner" [for Nurse Practitioner job description, see appendix] and engage in the expanded practice of nursing, which may include the performance of primary health care services and the issuance of prescriptions. Practice agreements and practice protocols must be established and maintained in the practice settings of the Nurse Practitioner and collaborating physician where they shall be available to the Department for inspection. 2. Practice Protocols Practice protocols shall identify the area of practice to be performed by the Nurse Practitioner in collaboration with the physician and shall reflect accepted standards of nursing and medical practice. Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. Often, FCS leads the nation in initiating research studies and offering ground-breaking new therapies to patients. Come join us today! Summary: Assists physician in the care and teaching of patients. PRIMARY TASKS AND RESPONSIBILITIES : Assess patients' physical and psychosocial status through interviews, health histories, physical examinations, and diagnostic studies. Write prescriptions for medication and blood products based on laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and care post-discharge in accordance with written practice protocols. Based on findings, determine patients' eligibility for entry into clinical trials. Interpret and evaluate findings of studies/tests. Relay appropriate information regarding patient care to the collaborating physician. Administer therapeutic measures / obtain specimens as prescribed. Dictate follow-up letters to referring physicians with summaries of treatment, response and plan. Dictate or writes patient history, admissions, progress notes, and discharge notes in a timely manner. Assist in the management of family dynamics and coping mechanisms during acute and chronic phases of patients’ care. Conduct or participate in daily multi-disciplinary rounds for assigned patients to ensure patients are receiving appropriate and quality care. Provide on-call coverage and perform hospital rounds as needed. Triage patients to determine the urgency of physician evaluation. Document and report care in an accurate, timely manner using appropriate forms and records. Demonstrate a willingness to learn and to remain flexible in the changing healthcare environment. Assist staff in times of crisis or emergency. Meet with the public outside the institution and efficiently market and promote cancer treatment. Determine work priorities, assign work and insure proper completion of work assignments. Comply with all federal and state laws and regulations pertaining to patient privacy, patient’s rights, personnel law, safety, labor, and employment law. Adheres to FCS and departmental policies and procedures, including IT policies and procedures and Disaster Recovery Plan. Other duties as requested. Keep work area and records neat and orderly. Maintain all company equipment in a safe and working manner. Maintain and ensure the confidentiality of all patient and employee information as required by the HIPAA regulations. Assist in training new employees to related job duties. Will be expected to work at any FCS location to help meet FCS business needs as directed by collaborating physicians. Will be expected to work overtime when given sufficient notice of required overtime. EDUCATION/CERTIFICATIONS & LICENSES: Graduate from an accredited Nurse Practitioner or Physician Assistant program, master’s degree in nursing or post-masters nurse practitioner certificate program 0-3 Years Licensed as Nurse Practitioner or Physician Assistant Current license/registration to practice in the state of Florida, prescriptive privileges or eligible to apply Certification from PANCE (Physician Assistant National Certifying Exam), American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the Oncology Nurses Certification Corporation (ONCC). BLS certified EXPERIENCE: Leadership skills necessary to direct others toward objectives that contribute to the successful treatment of patients. Analytical skills necessary to evaluate patients’ status, health care procedure / techniques and to monitor quality of patient care. Fiscal skills to monitor and control costs and revenue. Ability to exercise sound judgment and discretion while performing duties is essential.
