We are currently seeking an MDS Coordinator (LPN or RN) to join a skilled nursing team in Southern Colorado. This is a great opportunity for a nurse who enjoys care planning, documentation, collaboration, and making a direct impact on resident outcomes.
MDS or care planning experience is strongly preferred, but we are also open to experienced nurses who are eager to learn and grow in this area.

What You'll Do
Complete timely and accurate MDS assessments in accordance with federal and state regulations
Collaborate with the interdisciplinary team to develop individualized resident care plans
Monitor care plan implementation and coordinate services
Conduct resident interviews and gather comprehensive assessment information
Maintain accurate and compliant clinical documentation
Participate in care plan meetings and communicate with residents and families
Monitor changes in resident conditions and update care plans as needed
Stay current with MDS guidelines, Medicare/Medicaid requirements, and clinical reimbursement processes
Work closely with nursing, therapy, social services, dietary, and other members of the interdisciplinary team
What We're Looking For
Current RN or LPN license in Colorado or a compact state
Previous MDS, Care Plan, or similar clinical coordination experience strongly preferred
Knowledge of Medicare and Medicaid regulations related to MDS
Strong attention to detail and organizational skills
Excellent documentation and communication skills
Ability to work independently while collaborating with an interdisciplinary team

Strong commitment to quality care and regulatory compliance
