Job Summary

The Employee Health Nurse is responsible for implementing the Hospital Employee Health program by interacting with staff to ensure annual health requirements are current, including vaccinations, tuberculosis testing, respiratory mask fit testing, exposure follow-up and assisting with prevention, recognition, and treatment of illness and injuries. Provides education with focus on promotion and restoration of health and environmental safety for prevention of illness and injury.

Job Requirements

Minimum Education Requirement:

Graduate of Licensed Vocational Nursing School.

Minimum Experience Requirement:

One year of Employee Health clinical experience preferred. Proficient computer skills with knowledge of Microsoft Office systems required. Excellent customer service skills required.

Minimum License Requirement:

Maintains BLS certification. Current LVN Licensure. Willingness to pursue Worker's Compensation Claims Administration (WCCA) certification preferred.

Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $30.00 - $35.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. Schedule is 11pm-7:30am, Sat & Sun. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge. Link clients to medical services when necessary (insurance, PCP, specialists, referrals). Coordinate home health visits (if needed) and follow-up on medical appointments. Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health. Arrange transportation for clients to and from medical appointments, as needed Report emergency situations to Supervisor/Manager and document incident Seek proper level of care to assess and treat medical complication Initiate medical response system when appropriate Support patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility. Administrative/Record Keeping: Enter patient information and case notes into EHR and other documentation systems. Complete and update medication, vitals, progress notes and care plan Perform administrative tasks such as answering phones and filing paperwork. Prepare patient reports for billing. Maintain proper inventory of medical supplies and medications. Compliance/Safety: Maintain confidentiality in accordance with HIPAA guidelines, Illumination Foundation policies and DHS standards. Provide patients with proper compliance forms upon admission Utilize infection control measures according to OSHA standards. Conduct medication reconciliation and ensure proper disposal and pickup of bio-medical/pharmaceutical waste. Maintain a safe work environment by ensuring the cleanliness of the med room, wound care room, and medication storage room. Qualifications Required: Current and non-restricted LVN license in California 2 years of relevant experience. High School Diploma or equivalent.
