What You'll Do

Clinical Leadership: Lead, support, and supervise nurses, medication technicians, caregivers, and care aides while promoting accountability and teamwork.
Resident Assessments: Complete initial and ongoing resident health assessments and identify changes in condition or level-of-care needs.
Individualized Care Plans: Develop, review, and update resident service and care plans to ensure each resident’s needs are appropriately addressed.
Care Coordination: Coordinate resident services with physicians, pharmacies, home health, hospice, therapy, and other healthcare providers.
Family Communication: Serve as a trusted clinical resource for residents and families, providing updates and communicating changes in condition or care needs.
Regulatory Compliance: Maintain compliance with applicable state regulations, healthcare standards, infection-control practices, and community policies.
Admissions & Transitions: Participate in new resident assessments, admissions, hospital returns, and transitions of care to help ensure appropriate placement and continuity of services.
Collaborative Leadership: Partner closely with the Executive Director and community leadership team to support resident satisfaction, staffing, retention, compliance, and overall community success.

Assisted Living, senior living, or related healthcare experience preferred
