About the Role:

We are seeking a dedicated and compassionate LPN to join our team of skilled nursing professionals. As an LPN at Hudson Elms, you will be responsible for providing high-quality nursing care to our patients. This includes administering medications, monitoring vital signs, and assisting with daily living activities. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring our patients receive the best possible outcomes.

Responsibilities:

• Provide direct patient care to assigned residents, including administering medications, taking vital signs, and assisting with daily living activities
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor patient progress and report any changes or concerns to the nursing team
• Collaborate with other healthcare professionals to ensure seamless care coordination
• Maintain accurate and detailed records of patient care and treatment
• Participate in quality improvement initiatives to enhance the quality of patient care

Requirements:

• Current LPN licensure in the state of Ohio
• Minimum of 6 months of experience as an LPN (will consider new graduates)
• Ability to work effectively as part of a team
• Strong communication and interpersonal skills
• Ability to lift, transfer, and reposition patients as needed
