Job Summary

Does it bring you joy to help seniors navigate the complexity of the healthcare system? This position is PRN field work as a Care Manager, assisting seniors with coordinating their care between multiple providers and agencies. Enable senior citizens to live comfortably at home and bring peace of mind to family members knowing their loved ones are cared for. Serve as the expert to assist client with obtaining providers to meet their needs. Visit clients in their homes to conduct initial assessments and re-assessments, write client care plans, recommend and coordinate changes in the level of care, assist clients with understanding available care options and connecting them with providers. Update care plan and supervisory notes in electronic records. New clients may at times require short notice assessments within the company service area of Fairfax County, Arlington, and Alexandria. Reports to the Administrator.

Job Responsibilities

  • Care Management, assisting seniors with varied needs to ensure their best quality of life.
  • Duties depend on specific client needs, and may change over time. Accompany the client to doctor appointments, assist with coordinating client care between different providers. Make recommendations to client on what care options may best meet their needs, to include skilled care, therapy, PT, nutrition, and personal care.
  • Assist the client with understanding and implementing guidance from health care professionals.
  • Assist with keeping client family informed.
  • Maintain careful documentation of client case.
  • If desired, be available to assist with homecare client in-home assessments

Required Skills & Qualifications

  • RN with current license to practice in Virginia
  • Certification as CMC, CCM, or other Care Management certification, or the requisite experience to obtain Care Management certification.
  • Previous experience working in Homecare, Assisted Living, Home Health, Skilled Nursing Facility highly desirable.
  • Highly proficient communicator, comfortable with various communication methods including phone, email, Zoom, telehealth, etc.
  • Reliable transportation and willing to travel to client homes and appointments.
  • Experience with electronic client record management systems.
  • Flexibility and willingness to meet the unique needs of clients, caregivers, and family members.
  • Authorized to work in the United States for any employer
  • Consent to a Background Check and Drug Screen

The Registered Nurse is responsible for the delivery of patient care services through coordination, implementation, ad supervision of field staff. The Registered Nurse Supervisor ensures that field staff follows the POC and delivers care accordingly. H. Conduct new employee orientation and in-services as needed. K. Documents direct patient services, coordination and collaboration with physician and other disciplines or provides in the care of the patient, ensuring document is complete, and complies with acceptable home health standards and Agency policy. L. Documentation is timely and meets professional standards of practice, establishes compliance with state regulations and Medicaid conditions of participation. M. Reviews caseload and charts documentation to ensure compliance and assesses patient progress and effectiveness of care delivery in meeting patient goals. N. Ensures that MD orders are faxed timely, follow with MD offices to ensure timely receipt O. Conducts employee physical as needed P. Completes employee annual evaluations Q. F/U calls to clients/families and physicians on all clinical matters R. F/U calls to clients/families and referral sources to respond to complaints S. Collaborating with leadership/management team and office staff to facilitate timely office, processing of paperwork and personnel information T.
