Advanced Practice Provider, Neurocritical Care, Community Health Partners

Overview

Community Health Partners is seeking a full-time Advanced Practice Provider (APP) to join the Neurocritical Care team. In this role, the APP will provide comprehensive care for patients with critical neurologic illness, including acute stroke, intracranial hemorrhage, status epilepticus, and patients recovering from neurosurgical and neurointerventional procedures, in collaboration with the interdisciplinary care team. Responsibilities include conducting detailed clinical assessments, developing and managing treatment plans, prescribing appropriate therapies, and performing critical care bedside procedures. The APP will support patients throughout the critical care continuum and ensure high standards of safety and quality. This position follows a 7-on, 7-off schedule with day and night shifts. Night shift responsibilities are assigned based on experience and training.

 

Our Neurosciences department now includes the Bob Smittcamp Family Neurosciences Institute, a five-story, 60,000-square-foot facility on the Community Regional Medical Center campus that opened in May 2026. Funded by $30 million in community donations, the Institute brings together more than 60 physicians and 30 advanced practice providers across stroke and neurovascular care, epilepsy, neuro-oncology, traumatic brain and spine injury, movement disorders, and neuromuscular disease. Community Regional is certified by The Joint Commission as an Advanced Comprehensive Stroke Center, the highest level of stroke certification and the only one in the five-county region, with 24/7 access to on-site neurosurgical expertise. Our 30-bed Neurocritical Care Unit has also earned the AACN Beacon Award for Excellence at the Gold level.

 

In this role, you will:

  • Provides direct assessment, evaluation and primary patient care, complying with medical protocols and procedures.
  • Provides direct patient management in collaboration with physicians; participates in daily rounds, patient assessments and care plan development.
  • Orders, interprets and evaluates diagnostic tests to identify and assess patient's healthcare needs. Furnishes medications based upon written protocols.
  • Performs critical care bedside procedures, including central venous catheter placement, arterial line placement, lumbar puncture, and endotracheal intubation as credentialed.
  • Ensures that patients are counseled regarding treatment plans and the use, dosage and possible side effects of medications.
  • Makes referrals to appropriate providers when patients require subspecialty medical attention beyond the area's scope of service.
  • Documents and maintain precise and complete records of patients.
  • Distinguishes between normal and abnormal findings.
  • Provides education, training, information and consultation services to interdisciplinary team, patients, family and caregivers.

Qualifications

Education and Experience Minimum Requirements:

  • Completion of an accredited Nurse Practitioner or Physician Assistant program required
  • Credentialed by the CMC Medical Staff Office as an Advance Practice Provider

Licenses and Certifications:

If qualifying as a Nurse Practitioner, the following is required

  • NP - Current State of California Nurse Practitioner License required
  • NPF - Nurse Practitioner Furnishing Number required

If Qualifying as a PA, the following is required:

Position: Cardiac Advanced Practice Provider Monogram Health is seeking a Cardiac Advanced Practice Provider for the delivery of personalized, compassionate medical care to patients primarily with heart failure and other cardiac conditions. The Advanced Practice Provider will be responsible for caring for patients, maintaining accurate and current patient records and scheduling, administering follow-up appointments as required, and supporting the training and clinical readiness of local market APPs caring for patients with cardiac conditions in the home. The successful candidate will work as a team with our physician specialists, field-based nurses, community health workers, and physicians to deliver premium care to every patient. Primary duties include patient health assessment, creating strategies to improve or manage a patient’s cardiac condition, introducing habits for health promotion, and helping local market clinicians apply evidence-based cardiac care practices in the in-home setting. He/she may also conduct physical exams, order tests, prescribe medications, and serve as a coordinator with the patient's primary healthcare provider and their specialists. This position will not be office-based but will be remote in state in which employed and will need to attend periodic training/meetings outside of that state. Compensation The base salary ranges for this role are listed below by state and reflect the range of experience and qualifications we may consider. Final compensation will be determined based on geographic location and job-related factors such as experience, skills, education, certifications, and internal compensation considerations. While a larger range is posted, most offers will typically fall within a narrower band based on these factors.
