Monday through Thursday, 2:00 PM–7:00 PM—No weekends, No holidays, and No on-call.

About the Role:

 

This is a Skilled Case Management position—no clinical care required.

 

  • Schedule and perform Q60 day HHA Supervisory/Recertification visits

  • Schedule and perform Start of Care assessment visits

  • Ongoing Case Management for client and HHA oversight

  • Evaluate the client status and the skills of staff members

  • Complete timely and accurate clinical assessments within 3 days of the visit

  • Complete timely and accurate 485/ Plan of Care for patients within 3 days of the visit

  • Provides patient, family, or caregiver education

 

The role is Office based out of our Bethlehem Location with local travel, typically 3-4 home visits per day for brief assessments.

Monday through Thursday

2:00 PM–7:00 PM

 

NO weekends

NO holidays

NO on-call

Qualifications

  •  Active RN License in Pennsylvania

  • Valid Driver’s License and reliable vehicle

  • Personal laptop

  • PPD or QuantiFERON test

  • CPR Certification

  • Minimum 1 year of nursing experience (home health experience preferred)
