LPN/RN MDS Coordinator

About the Role:

We're looking for a highly motivated and organized LPN/RN MDS Coordinator to coordinate the resident assessment process, identify individualized care needs, and develop individualized care plans. As a key member of our care team, you'll work closely with our licensed nurses, therapists, and other healthcare professionals to ensure the delivery of high-quality care to our residents. Responsibilities:

Coordinate the resident assessment process, including scheduling and conducting MDS assessments, and developing individualized care plans
Identify individualized care needs and communicate with the interdisciplinary care team to develop and implement comprehensive care plans
Collaborate with licensed nurses, therapists, and other healthcare professionals to ensure the delivery of high-quality care to our residents
Maintain accurate and up-to-date records of resident care, including assessment results, care plans, and progress notes
Participate in facility-wide quality improvement initiatives to promote resident-centered care and improve resident outcomes
Provide education and support to family members and caregivers on resident care and services

Requirements:

Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Missouri
Experience in long-term care or related field preferred
Strong organizational and communication skills
Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines
Strong knowledge of MDS and RAI processes
