Job Summary The Licensed Practical Nurse (LPN) provides general nursing care to patients, families and caregivers in the Medical Group practices. The Licensed Practical Nurse (LPN) assists and advocates for the patient and families health in support of the patient centered approach and delivery model of care, and is an active care team member. Responsible for collaborating with physicians/ providers , registered professional nurses and multidisciplinary care team members. A Licensed Practical Nurse (LPN) can perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider. Nursing shall perform in accordance with the standards outlined for Licensed Practical Nurse (LPN) in the NYS Education Law, NYS Office of the Professions, and by the State Nurse Practice Act. A Licensed Practical Nurse  will follow the mission, vision, I-CARE values, policies and procedures of Mohawk Valley Health System.
