Providing direct patient care under the supervision of licensed nursing staff, including administering medications, collecting vital signs, and performing treatments
Developing and implementing individualized care plans in collaboration with the interdisciplinary team
Maintaining accurate and detailed records of patient care and progress
Assisting with daily living activities, such as bathing, dressing, and grooming
Participating in shift reports and communicating effectively with other members of the healthcare team

Requirements:

Current LPN license in the state of Ohio
High school diploma or equivalent required; associate's degree in nursing or related field preferred
Minimum of one year of experience as an LPN; experience in rehabilitation or long-term care setting a plus
