What You’ll Do & Key Responsibilities

  • Provide direct, compassionate nursing care to residents in a skilled nursing environment.
  • Support daily clinical operations on the unit, ensuring quality care and resident safety.
  • Administer medications and treatments accurately and on time.
  • Monitor resident conditions and respond promptly to changes in status.
  • Collaborate with CNA, LPN, provider, and other interdisciplinary team members to promote teamwork and high-quality care.
  • Communicate effectively with residents and families regarding care needs.

  • Follow infection control, safety, and compliance protocols at all times.

Qualifications

  • Valid RN license in good standing. License must be valid within the state of Iowa, or ability to activate.
  • Current, valid CPR Certification.
  • Knowledge of federal and state long-term care regulations and laws.
  • A strong commitment to helping others and consistently treating them with empathy, respect, patience, and discretion.

This role requires working four 10-hour shifts, ideally Tuesday through Friday, from 8:00 a.m. to 6:00 p.m. Key Responsibilities Of The Hospice Admissions Nurse Provide nursing services to Care Initiatives Hospice patients, through visits that may include telephonic and in-person management of patients, families, caregivers and referral or community sources. Establish positive relationships with community sources with the intent to enhance and expedite patient admissions and coordination of care, medications, equipment, and supplies. Determine immediate needs and initial assessment, share findings with the RN-Case Manager and collaborate care with the IDT. Provide visits to assess ongoing patients’ needs, revise care plans as needed and communicate patient assessments to the team, when not doing admissions. Provide clinical services to patients and families that address goals of the care plan. Prepare clinical and progress notes; maintain clinical records; coordinate services and inform physician and other personnel of changes in patient condition and needs.
