Job Summary:

Unity Health and Rehabilitation Center, Setting standards in Rehabilitation and Skilled Nursing Care, is seeking a dedicated, compassionate and experienced MDS Coordinator to join our team. As a vital member of our care delivery team, you will be responsible for ensuring the highest quality of care for our long-term care patients.

About the Role:

As an MDS Coordinator, you will be responsible for:

  • Collecting and analyzing data to develop a comprehensive care plan for each patient

  • Coordinating with care providers, therapists, and other healthcare professionals to ensure seamless care delivery

  • Assisting with the development and implementation of individualized treatment plans

  • Monitoring patient progress and making adjustments to care plans as necessary

  • Maintaining accurate and timely records of patient care and treatment

Responsibilities:

  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans

  • Gather and analyze data to identify trends and areas for improvement in patient care

  • Develop and maintain accurate and timely records of patient care and treatment

  • Assist with the implementation of individualized treatment plans and monitor patient progress

  • Participate in quality improvement initiatives to enhance patient care and outcomes

  • Maintain effective communication with patients, families, and healthcare team members

Requirements:

  • Current certification as a Licensed Practitioner (LPT) or Registered Nurse (RN) in the state of Florida

  • Minimum of 1 year experience in a long-term care setting, as an MDS Coordinator

  • Strong analytical and problem-solving skills

  • Excellent communication and interpersonal skills

  • Ability to work effectively in a fast-paced environment

  • Familiarity with MDS software and electronic medical records
