Definition of Position: 

The Registered Nurse specializing in perioperative practice performs nursing activities in the pre-anesthesia, post anesthesia Phase l, and post anesthesia Phase Il care of the patient's surgical experience. The recovery RN monitors patients having “local only” procedures in the OR and may administer conscious sedation, if certified.  Works to ensure provision of quality care of pre and post procedure patients through utilization of the nursing process, established standards of care, and policies and procedures. The peri anesthesia nurse acts as the patient advocate and is responsible for the direction and supervision of any personnel assigned. Will be on call for the Post-Anesthesia Unit on a regular scheduled rotation.  The employee supports the hospital mission, vision, values, policies, and procedures.


Responsibilities

Essential Job Functions:

  • Completes nursing Assessments on patients in the pre and postoperative phase of care.
  • Performs patient admissions according to hospital standard of care.
  • Initiate and manage intravenous access.
  • Closely observe patients for changes in heart rate, respiratory rate, oxygen saturation, blood pressure, and consciousness. Report and intervene as needed based on medical guidelines.
  • Uses the Aldrete scoring system in PACU phase of care
  • Ensure the patient maintains a patient airway and adequate respiratory function.
  • Perform interventions to maintain the status of the patients’ genitourinary, gastrointestinal, musculoskeletal, endocrine, respiratory, cardiovascular, neurological systems, and fluid and electrolyte balance.
  • Inform medical staff about the patient's status consistently.
  • Administer pain medications and assess their effectiveness.
  • Treat post-operative nausea using medications and comfort measures.
  • Develop an individualized peri anesthesia plan of care.
  • Assists anesthesia with various nerve blocks as needed.
  • Administration of medications using the 8 rights of safe medication practices.
  • Assess and perform interventions to maintain the integrity of the patients’ surgical sites to enhance tissue perfusion and promote infection prevention.
  • Performing interventions to maintain normothermia throughout the continuum of care
  • Evaluate the patient's and their designated caretaker's knowledge level. Provide education on expected outcomes, medication management, wound care, diet, activity, anesthesia side effects, when to seek medical care, signs and symptoms of complications, and any other necessary educational needs.
  • Assess patients’ progress towards desired outcomes.
  • Collaborate effectively with other disciplines and nursing staff as needed.
  • Transfer patient care as needed
  • Delegate tasks by Oregon State Board of Nursing guidelines.
  • Use critical thinking, problem-solving, and conflict resolution skills to build effective team relationships.

Qualifications

Qualifications:

Education

                Required: Associate of Applied Science in Nursing.

                Preferred: Bachelor of Science in Nursing.                                                                                 

Licenses/ certifications/ registrations

Required: Current Registered Nurse License in the State of Oregon, BLS/PALS/ACLS, & TNCC.

Preferred: Conscious sedation certification, CAPA/ CPAN certification, Ultrasound Guided IV certification.

Experience

Required: One-year Critical care experience or two years medical surgical experience.

Preferred: Previous Day Surgery, PACU, Pre-admission services, or ambulatory surgery center experience.  Familiar with glucometer, art lines, space labs, epic charting, bladder scanner, ultrasound machines, Belmont rapid infuser, blood warmers.

The RN is responsible for pre-procedure, intra-procedure, and post-procedure patient care throughout the interventional patient experience. The Interventional RN ensures safe, efficient, and high-quality care using the nursing process, established standards of care, and organizational policies. The RN collaborates with physicians and the interdisciplinary team, serves as a patient advocate, and participates in call coverage as required. Responsibilities Essential Job Functions : Pre-Procedure Care Perform pre-procedure nursing assessments and review relevant clinical information. Verify patient identity, procedure, NPO status, medication reconciliation and consent according to institutional policy. Initiate intravenous access and prepare patients for procedures. Collaborate with the procedural team regarding case planning. Document required information in the electronic medical record (EMR). Provide patient and family education regarding the procedure and recovery expectations. Intra-Procedure Care The Interventional RN provides continuous patient monitoring and clinical support during procedures and may function in one of the following roles. Sedation RN Administer procedural medications per performing provider order Administer and monitor moderate sedation in accordance with organizational policy and credentialing requirements. Perform a pre-sedation assessment, including verification of allergies and NPO status, and ensure that the airway evaluation and ASA classification are documented in accordance with policy. Ensure laboratory results are within limits for specific procedure, communicating with performing provider results outside of limits and potential need for additional testing. Continuously monitor patient status including cardiac rhythm, blood pressure, respiratory status, oxygen saturation, and level of consciousness. Maintain airway patency and respond to sedation-related complications. Communicate patient status with the procedural team and document medications and monitoring parameters in the EMR. Circulating RN Coordinate nursing care in the procedure room and ensure completion of patient identification, procedure verification, and procedural time-out. Maintain a safe procedural environment and assist with patient positioning and monitoring setup. Assist in administration of medications as needed per performing provider order Assist with vascular access site management when appropriate. Recognize and respond to procedural complications such as bleeding, arrhythmias, or respiratory compromise. Communicate patient status, manage specimens, and document care and procedural events in the EMR. Procedural Monitoring RN Functions as the primary documenter, recording hemodynamic data, pressures, and procedural events. Ensures allergy considerations are communicated during the timeout and reflected in procedural documentation. Operates and troubleshoots physiological monitoring systems (e.g., hemodynamic recorders, ECG, oxygen saturation). Documents all supplies used, implants, contrast usage, and procedural details accurately. Communicates real-time findings to the procedural team, ensuring accurate documentation of events, medications, and device deployment. Post-Procedure Care Receive and recover patients following procedures and perform post-procedure assessments including vital signs, neurological status, and access site evaluation. Maintain airway patency and monitor for hemodynamic or respiratory changes. Manage pain, nausea, and other recovery needs. Assess and manage vascular access sites and maintain hemostasis. Administer medications and treatments as ordered. Educate patients and caregivers regarding recovery, medications, activity restrictions, and signs of complications. Coordinate patient transfer or discharge and provide appropriate handoff communication. Additional Responsibilities Patient Advocacy and Collaboration Advocate for patient safety and rights while maintaining patient confidentiality. Communicate effectively with patients, families, and the interdisciplinary team. Delegate tasks appropriately in accordance with Oregon State Board of Nursing guidelines. Department Support Assist with maintaining supplies, equipment, readiness, and room turnover. Participate in quality improvement and regulatory programs including DNV requirements. Maintain required competencies and continuing education. Qualifications Qualifications: Education Required: Graduate of an accredited school of professional nursing. Evidence of continuing education appropriate for skill level and area of practice. Preferred: Bachelor of Science in Nursing (BSN) Licenses/ certifications/ registrations Required: Current RN license in the State of Oregon; current BLS, ACLS certification, and NIH Stroke Scale Certification. Preferred: Certifications specific to areas of clinical specialty preferred (e.g., RCIS, CRN, CV-BC, CAPA or CPAN), IV certification, Moderate Sedation Certification. Experience Required: One year of Interventional or Operating Room nursing experience. Preferred: Experience in Interventional Radiology, Cardiac Cath Lab, PACU, or ambulatory surgery. Other: Infusion pumps, PCA pumps, and other medical equipment, computer, printer, photocopier, and patient charts. Ability to provide care for the patient's age-specific, cultural, physical, and psychosocial wellbeing. The role requires the ability to lift to 50 pounds independently and involves frequent pushing, pulling, carrying, or repositioning of objects or patients weighing up to 25 pounds. There may also be occasional lifting or transferring of individuals weighing over 100 pounds, with the expectation that appropriate lifting techniques, mechanical aids, or team assistance will be used to ensure safety. May to sit for long periods of time. Working Conditions: This position is performed primarily in an indoor healthcare environment, such as a hospital, clinic, or long-term care facility. While the work setting is protected from outdoor weather conditions, it may be subject to variable indoor temperatures. The role involves frequent exposure to infectious and contagious diseases, hazardous materials, and medical equipment, with the expectation that all staff follow appropriate safety protocols and utilize personal protective equipment (PPE) as required.
