Position Summary

The Licensed Practical Nurse (LPN) plays a key role in supporting the health and well-being of residents in Assisted Living and Memory Care. This position is responsible for clinical assessments, medication administration, care planning, and ensuring residents receive high-quality, individualized care while maintaining dignity and independence.

Key Responsibilities

Administer medications and treatments in accordance with physician orders and state regulations
Conduct resident assessments and assist in care planning and updates
Monitor resident conditions and communicate changes to the care team
Ensure accurate and timely documentation of nursing services
Support and supervise caregivers and med techs as needed
Promote a safe, clean, and compliant care environment

Qualifications

Active Licensed Practical Nurse (LPN) license in the state of Massachusetts required
Previous assisted living, memory care, or long-term care experience preferred
Strong knowledge of medication administration and clinical best practices
Excellent communication and teamwork skills
Ability to work in a fast-paced environment while maintaining attention to detail

Physical Requirements

Ability to stand, walk, bend, and assist residents for extended periods
