Position Overview
Aspire Senior Living Roaring River is seeking a compassionate, dependable, and clinically skilled Licensed Practical Nurse (LPN) to join our nursing team.
The LPN provides direct nursing care and clinical support while helping promote the health, safety, comfort, dignity, and overall well-being of our residents. Working alongside RNs, CNAs, CMTs, physicians, and other members of the interdisciplinary team, the LPN plays an important role in medication administration, treatments, resident monitoring, documentation, and care coordination.
We're looking for an LPN who combines strong nursing skills with a genuine desire to build relationships and make a difference in the lives of older adults.
What You'll Do
Resident Care & Nursing Assessment
Provide direct nursing care according to resident needs, established care plans, provider orders, and the LPN's scope of practice.
Perform and document resident observations and nursing assessments as permitted by applicable regulations and facility policy.
Monitor residents' physical, emotional, and psychosocial status.
Recognize and promptly report significant changes in resident condition to the appropriate RN, physician, provider, or nursing leadership.
Implement nursing interventions and treatments according to established plans of care.
Help ensure residents receive compassionate, individualized, and person-centered care.
Medication & Treatment Administration
Administer medications and treatments according to provider orders and established medication-administration procedures.
Monitor residents for medication effectiveness, side effects, adverse reactions, and other clinical concerns.
Accurately document medication administration, treatments, vital signs, and resident responses.
Follow established medication safety and infection-prevention practices.
Communicate medication or treatment concerns promptly to the appropriate healthcare provider or nursing leader.
Provide resident and family education regarding medications and treatments within the LPN's scope of practice.
Wound Care & Clinical Procedures
Provide wound care, dressing changes, skin treatments, and other nursing procedures within the LPN's scope of practice.
Monitor skin integrity and identify potential concerns.
Document wound characteristics, treatments, and resident response as required.
Communicate changes in wound condition or healing progress to the appropriate clinical team member.
Collaborate with physicians, wound-care professionals, RNs, and other interdisciplinary team members as appropriate.
Follow infection-prevention procedures during all treatments and clinical care.
Resident & Family Support
Build positive and trusting relationships with residents and their families.
Provide education, reassurance, and compassionate support regarding resident care and treatment.
Encourage resident participation in care decisions whenever appropriate.
Respect and advocate for resident rights, dignity, privacy, preferences, and independence.
Support residents and families through changes in condition, chronic illness, rehabilitation, and other healthcare needs.
Clinical Leadership & Teamwork
Work collaboratively with RNs, CNAs, CMTs, physicians, Therapy, Social Services, Dietary, and other members of the interdisciplinary team.
Provide direction and support to nursing assistants and other team members as appropriate and within applicable regulations.
Help ensure assigned resident-care responsibilities are completed safely and appropriately.
Serve as a clinical resource and positive role model for nursing team members.
Promote teamwork, accountability, professionalism, and effective communication throughout the nursing department.
Documentation & Communication
Maintain accurate and timely nursing documentation in the electronic health record (EHR).
Document resident observations, treatments, medications, interventions, and responses according to established requirements.
Participate in shift handoffs, care conferences, and interdisciplinary meetings as appropriate.
Communicate important resident information and changes in condition to appropriate team members.
Maintain resident confidentiality and protect health information at all times.
Safety, Infection Prevention & Compliance
Follow applicable nursing standards, facility policies, and federal and state requirements.
Follow infection-prevention protocols and standard precautions.
Utilize appropriate personal protective equipment (PPE) when required.
Support fall prevention, resident safety, emergency preparedness, and other clinical safety initiatives.
Identify and promptly report potential hazards, incidents, or resident-safety concerns.
Support continuous survey readiness through accurate documentation and consistent nursing practices.
Perform other duties as assigned.
What We're Looking For
Current, unrestricted Licensed Practical Nurse (LPN) license in the state of Missouri.
Graduate of an approved practical nursing program.
Previous experience in long-term care, skilled nursing, rehabilitation, senior living, or geriatric nursing preferred.
New graduate LPNs may be considered based on community needs and applicable requirements.
Knowledge of nursing principles, medication administration, treatments, infection prevention, and resident-care practices.
Wound-care experience preferred.
Strong clinical observation, critical-thinking, and problem-solving abilities.
Ability to recognize and appropriately report changes in resident condition.
Strong written and verbal communication skills.
Ability to work effectively with residents, families, physicians, and interdisciplinary team members.
Experience with electronic health records (EHR) preferred.
Ability to prioritize responsibilities and manage time effectively in a fast-paced healthcare environment.
