Then you will plan, organize, and assist with home health care service with your focus on just one patient for your shift. As a Skilled Nurse (Registered Nurse or License Practical Nurse

You will assist patients with daily living activities, ensure their comfort, and support their overall well-being to enhance their quality of life.

Responsibilities may include the following:

  • Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
  • Demonstrate understanding organizational and nursing policies and procedures.
  • Demonstrate strong communication skills, both spoken and written.
  • Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
  • Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
  • Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
