About the role

An established supportive living community in Lake County, IL is hiring a Wellness Director to lead its nursing and wellness department. You will oversee resident care, supervise the clinical team, and keep the community survey-ready. RN preferred; experienced LPNs are encouraged to apply.

This is a hands-on leadership role for a nurse who wants autonomy, a manageable resident population, and real support from regional clinical leadership.

What you'll do

  • Direct day-to-day resident care and wellness services

  • Complete and oversee resident assessments, service plans, and change-of-condition follow-up

  • Hire, train, schedule, and supervise nurses and CNAs

  • Oversee medication management, including order processing, audits, and pharmacy communication

  • Maintain compliance with Illinois Supportive Living Program regulations and lead survey preparation

  • Communicate with physicians, families, and outside providers such as home health and hospice

  • Lead staff in-services and competency checks

  • Monitor quality outcomes, including falls, infections, and hospitalizations, and drive improvement

  • Share in the on-call rotation for clinical questions

What we're looking for

  • Current Illinois RN license (preferred) or LPN license in good standing

  • 2+ years of nursing experience in assisted living, supportive living, long-term care, or skilled nursing

  • 1+ year of supervisory or nurse management experience

  • Working knowledge of state regulations and the survey process

  • Strong assessment, documentation, and communication skills

  • Comfort with electronic health records
