Responsibilities for RN Case Manager:

  • Coordinate and manage care for patients with chronic, acute, or complex medical conditions across the care continuum.

  • Conduct comprehensive patient assessments to identify physical, psychosocial, and environmental needs.

  • Develop, implement, and continuously evaluate individualized care plans in collaboration with interdisciplinary healthcare teams.

  • Monitor patient progress toward established goals and adjust care plans based on clinical outcomes and changing needs.

  • Serve as a patient advocate by ensuring the resident's needs, goals, and preferences are communicated and respected throughout the care process.

  • Facilitate communication between patients, families, physicians, and healthcare providers to ensure continuity and quality of care.

  • Coordinate medical appointments, referrals, transportation, and access to community resources as needed.

  • Ensure efficient and appropriate utilization of healthcare resources while maintaining high-quality care standards.

  • Provide education to patients and families regarding diagnoses, treatment options, and care planning.

Qualifications for RN Case Manager:

  • Minimum of an RN associate’s degree in nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.

  • Previous case management or utilization review experience preferred.

  • Direct patient care experience strongly preferred.

  • Strong communication, negotiation, and problem-solving skills.

  • Solid understanding of healthcare regulations, standards of practice, and care coordination principles.

Gardena Convalescent Center is a 24-hour skilled nursing facility conveniently located in a safe, residential neighborhood near Memorial Hospital of Gardena. Our 74-bed private and semi-private room facility is surrounded by beautiful gardens and enclosed patios, which add to the warm, homelike setting. We are seeking a full time Admitting nurse/desk nurse for 3-11 shift. RN preferred. Position is Monday thru Friday. A nurse having good rapport with staff, guests and family members with excellent verbal skills, good customer service standards. Must be able to monitor resident progress and identifies any changes in status, acting on those changes to insure resident comfort and safety, use critical thinking in emergencies, comply with all facility policy and procedures. The RN is responsible and accountable for the overall health status of the residents, including: assessment, evaluation, plan of care, monitoring and reporting changes to the physician and family. The RN may delegate selected nursing activities to other health care workers, but retains accountability and responsibility for the overall nursing care. Duties of Hospice Registered Nurse Include: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness. Conducts home visits and assesses/evaluates patient's status.
