As a hospice Registered Nurse Case manager, you will perform ongoing patient assessments, maintaining thorough and diligent notes and records regarding the effectiveness of each patient's care plan. You will also keep in constant contact with patient family members and alternative caregivers. Additional responsibilities for the hospice RN Case Manager will include: Providing nursing care with an emphasis on pain and other management. Supervising Home Health Aides. Referring patients' families to applicable community resources. Maintaining strict patient confidentiality. Providing patient care directives to family members and alternative caregivers. Requirements – As a hospice Registered Nurse, you must have absolute integrity and sound judgment. You will act as a patient advocate, working independently to ensure your patients are physically comfortable and content. We are seeking an RN with good social skills, energetic, who is organized and emotionally capable of caring for terminally ill individuals. You must also be a team player, able to work harmoniously with a wide variety of individuals. Additional requirements for the hospice RN Case Manager include: Licensed as a Registered Nurse in the state of Pennsylvania. Associate/Bachelor's/Masters Degree in nursing. Have a minimum of 2 years Med-Surg., Home Care, and/or Hospice experience. Understand the difference between Progressively Declined and Custodial Care. Ability to occasionally lift up to 25 lbs. The Full Time RN Case Manager provides nursing care with special emphasis on control of pain and other symptoms to the hospice patient. Implements physician plan of care and establishes a nursing plan of care based upon nursing process. Trains, supervises and supports the family in provision of care to the patient. Job Functions: Assess the physical and psychological status of the patient. Reviews past and present medical history of the patient. Perceptive in planning and problem solving. Provides emotional support to caregivers. Able to work independently and make critical decisions based on available patient information. The Patient Care Manager is a member of the interdisciplinary team. This position oversees the day to day clinical operation of hospice in conformance with the organization's mission, philosophy and goals and ensures that services conform to all applicable laws, rules and regulations. Reports to: Program Director Qualifications: Licensed as a Registered Nurse in PA . Education: A Registered Nurse with a Baccalaureate or higher degree preferred in nursing or other health related field. And two year’s clinical experience with the last three (3) years in a home health agency, home care setting or health facility.
