We are looking for an RN to join our team and support patients throughout Westchester, Oak Brook, Brookfield, Maywood, Lombard, and the surrounding areas.

 

As a Registered Nurse Home Health, you’ll contribute to our success in the following ways: 

  • Assessing patients to determine their care needs.  
  • Completing initial and ongoing patient assessments. 
  • Protecting patient and staff privacy in accordance with HIPAA.  
  • Providing nursing care according to the plan of care and physician orders.  
  • Monitoring patient conditions and reporting changes to the care team.  
  • Educating patients and families on their care and safety.  
  • Taking appropriate action when patient conditions change.  
  • Helping ensure quality, safe, and continuous patient care. 

What is Required?

  • Current State license as a Registered Nurse.  
  • Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.  
  • 1+ year of experience as a nurse in a clinical care setting.  
  • Home Health experience is preferred but not required.   
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment  
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

The base salary for this position is $83,000 to $93,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.

SUMMARY: The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission.
