Responsibilities

Work Schedule:

Monday - Friday, between the hours of 6:00am and 6:30pm, to include a lunch breakNo Weekends, No HolidaysNo Call, No Call-Back

 

Core Registered Nurse Duties: The duties include, but are not limited to the following;

  • Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost
  • Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
  • Lead and direct the entire team in quality patient care.
  • Ensure adequate staffing and supplies for clinic functions.
  • Manage appointment schedules, lead team huddles, lead the management of telephone and electronic messages.
  • Perform physical exam and health
  • Provide health promotions, counseling, and
  • Follow standing orders.
  • Administer medications as ordered by the provider. Assist with procedures and be available as a chaperone as necessary.
  • Assist with ordering and tracking completion of laboratory studies and medication as necessary per Clinical Protocols.
  • Ensure effective and efficient patient flow; develop, train, evaluate and provide feedback to Ancillary Nursing Personnel.
  • Triage patients that present unexpectedly for appointments via approved Clinical Protocols.
  • Recognize adverse signs and symptoms and quickly react in emergency
  • Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and
  • Follow up with patients after emergency room visits or inpatient hospitalizations, triage patients and schedule follow up appointments as needed.
  • Assist with ordering and tracking completion of patient referrals and arrange specialty care as appropriate.

Qualifications

Minimum Qualifications:* Education: Possess a Baccalaureate Degree in Nursing, a Graduate Diploma in Nursing, or an Associate Degree in Nursing, from an accredited agency such as the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)..* Experience: Possess a minimum of 1 year of full-time experience (36-44 hours per week) experience within the last 2 years.* Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse as required in the TO.* Life Support Certification: Possess and maintain a current American Heart Association BLS Healthcare Provider certification. * Security: Must possess ability to pass a Government background check/security clearance.

 

Pay Scale: $55/hr to $64/hr, to be determined based on employment options, qualifications, experience, and location

POSITION DESCRIPTION Responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the Director of Nursing and VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.
