About the Role:

As a Licensed Practical Nurse (LPN) at St. Monica Center for Rehab and Healthcare, you will play a vital role in delivering exceptional patient care to our residents. In this full-time position, you will provide direct patient care, including administration of medications, wound care, and other medical treatments under the supervision of our medical team.

Responsibilities:

• Deliver high-quality patient care by assessing patients' physical and emotional needs, developing individualized care plans, and implementing interventions to promote optimal health outcomes.
• Collaborate with interdisciplinary team members, including physicians, nurses, and therapists, to ensure comprehensive care delivery.
• Administer medications and treatments as prescribed by physicians and in accordance with hospital policies and procedures.
• Maintain accurate and up-to-date patient records, including detailed notes and reports.
• Participate in quality improvement initiatives to continuously improve patient outcomes and care delivery.

Requirements:

• Current LPN licensure in the state of Pennsylvania.
• Basic Life Support (BLS) certification.
• Ability to lift, transfer, and move patients, with or without assistance.
