Responsibilities:

Provide one-on-one care to clients in their homes, including assistance with daily living activities, wound care, and medication management
Support clients with mobility issues, such as transferring, ambulating, and using assistive devices
Assist with meal preparation, light housekeeping, and laundry
Maintain a safe and clean environment for clients, adhering to infection control procedures and hazard prevention strategies
Communicate effectively with clients, families, and healthcare professionals to ensure seamless care transitions
Monitor and report changes in client condition, and implement emergency procedures as necessary
