Job Responsibilities

  • Delivering post-acute care as an LPN
  • Monitoring the physical and mental wellbeing of residents and tracking their vital signs
  • Performing intakes of residents and documenting their health concerns and medical histories
  • Reviewing the medical records and charts of residents
  • Properly administering (and verifying) medications
  • Helping to maintain appropriate levels of medical supplies
  • Contributing to a safe, comfortable environment for residents
  • Assisting with IV set up, vaccinations, and phlebotomies
  • Being accountable to follow and adhere to care plans for residents

Qualifications and Skills

  • Current Ohio state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • Degree from an approved LPN training program
  • At least 6 months of experience (LPN or similar role)
  • Ability to lift a minimum of 50 lbs

Responsibilities & Requirements Under the supervision of the provider and/or practice manager, assists and delivers patient care duties, and provides administrative support throughout the practice. This position assists the provider in examination and treatment of patients, performing miscellaneous delegated duties under the direction of provider. MA/CMA : Current Medical Assistant certification/registration as CCMA, CMA (AAMA), NCMA, RMA (through AMT) or completion of KHMG Medical Assistant Training Program or MA program through an accredited institution of higher learning and successful attainment of certification/registration within 15 months of hire. LPN : Graduate of accredited LPN program (or equivalent) with current Ohio license. Medical assistants who are not certified or registered at the time of hire will be restricted from administering medication and entering Epic orders until the required credentials are obtained and verified.
