Job Description Housing Case Managers are responsible for providing comprehensive case management services to individuals experiencing homelessness or at risk of homelessness. This role involves a combination of advocacy, direct support, and resource coordination to meet the varied needs of clients. Key responsibilities include conducting assessments, implementing harm reduction strategies, delivering trauma-informed care, intervening in crises, and coordinating resources. The goal is to empower clients to attain self-sufficiency by equipping them with essential life skills and support needed to achieve housing stability and secure permanent housing. Attend Interdisciplinary Care Team meetings on a weekly basis to evaluate client goals and track the completion of action items. Monitor progress by utilizing the Electronic Health Record (EHR) and Homeless Management Information System (HMIS) databases to uphold accurate client information and program data. Link clients to resources to address psycho-social and daily needs such as health care, food, hygiene items. Perform crisis intervention, as necessary, and counsel client with confronting issues adversely affecting their lives, such as mental illness, addiction and abuse. Enroll families & individuals into the Homeless Management Information System through the Coordinated Entry System (CES) Housing Case Managers are required to attend training sessions to remain informed about housing laws, trauma-informed care practices, and harm reduction strategies annually. Advocacy and Landlord Engagement: Networking with landlords/property managers and other community entities. Conduct health and housing quality standards (if applicable) walk through of prospective units. Confirm the client’s move-in costs and collect all supporting documents, including but not limited to the W-9 form and the signed rental agreement. Understand fair housing laws, local housing laws and various housing programs within the community General understanding of community supports programs Confidentiality and Professionalism: Act in an ethical manner reflecting core values of integrity, transparency, accountability, respect and responsibility.
