Full Time Registered Nurse Case Manager Territory: Placer County* What you’ll do as a Registered Nurse (RN) Home Health Case Manager: Utilize your outstanding clinical skills as you create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
