Position Hours: Three (3) 12-hour shifts 6:45 p.m. - 7:15 a.m. Rotating based on unit need. No specific workdays or days off can be guaranteed. We cannot guarantee accommodation of school schedules. This client requires full assistance with activities of daily living, including hands-on transfer assistance, and is considered a significant fall risk. We need a caregiver who understands that fall prevention requires constant awareness, close supervision, careful assistance, and proactive safety measures throughout the entire shift. The selected caregiver must be available to work all three scheduled days Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Consistent attendance is essential because the client depends on a stable routine and familiar caregivers. The client also has a small dog in the home. Applicants must be comfortable entering and working in a home with a small household pet. At Helping Hearts, caregiving is more than a job, it is a calling. We are looking for someone who will treat the client with dignity, patience, compassion, and respect while remaining actively engaged and focused on the client’s safety. Primary Responsibilities Full Assistance with Activities of Daily Living The caregiver will provide respectful, hands-on assistance with the client’s daily personal-care needs as outlined in the client’s care plan. Safe Transfer Ability Is Required This client requires hands-on assistance with transfers. Applicants must have the physical ability and willingness to safely assist the client with transfers, including: Wheelchair to commode and commode to wheelchair Wheelchair to bed and bed to wheelchair Wheelchair to chair or other approved seating Sit-to-stand and stand-to-sit transfers Toileting-related mobility and clothing-management assistance Repositioning for safety, hygiene, and comfort Transfers using approved equipment when required by the care plan Previous experience using a Hoyer lift, sit-to-stand lift, or other mechanical transfer equipment is preferred but not required. Applicants without this experience must be willing and able to complete all required hands-on training and demonstrate safe use of the equipment before independently performing mechanical-lift transfers. During every transfer, the caregiver must: Explain the transfer process to the client Make sure the client is positioned correctly Properly position the wheelchair or commode Lock the wheelchair brakes before beginning Move footrests, cords, furniture, and other obstacles out of the way Clear and inspect the transfer area Use proper body mechanics Use approved transfer equipment when required Closely monitor the client’s balance and comfort Move slowly and avoid rushing or pulling the client Stop immediately if the transfer becomes unsafe Transfers must never be rushed or attempted in an unsafe manner. The caregiver must follow the client’s care plan and use any required gait belt, Hoyer lift, sit-to-stand lift, or other approved transfer device only after receiving proper training and authorization. If a transfer requires two caregivers or cannot be completed safely, the caregiver must not attempt it alone. The caregiver must keep the client safe and contact the Helping Hearts office immediately for assistance and further direction. Applicants must be able to perform the essential transfer responsibilities of this position safely and consistently, with or without reasonable accommodation. Proactive Fall Prevention and Safety Monitoring Because this client is at significant risk for falls, the caregiver must remain alert, attentive, engaged, and safety-focused throughout the entire shift. This is not a passive companion assignment or a position where the caregiver can sit nearby without actively observing and assisting the client. The caregiver will be expected to: Remain aware of the client’s location, mobility, and immediate needs Provide close supervision during standing, walking, toileting, and transfers Stay within a safe assisting distance whenever the client is moving Encourage the client to request assistance before attempting to stand or transfer Maintain a clear and hazard-free environment Check for spills, cords, loose rugs, clutter, poor lighting, and unsafe furniture placement Make sure mobility devices are properly positioned and within reach Make sure the client has appropriate footwear during mobility Keep commonly used items safely within the client’s reach Remain especially attentive when the client is tired, weak, distracted, or attempting to toilet Never leave the client unattended during a task when supervision is required Avoid personal-phone use or other distractions that interfere with active supervision Immediately report changes in mobility, balance, strength, or transfer ability Immediately report every fall, near fall, injury, or safety concern Complete all required incident documentation accurately and promptly The caregiver must never attempt an unsafe lift or transfer. If the client cannot be moved safely according to the care plan, the caregiver must protect the client from further injury and contact the Helping Hearts office immediately for direction. Active Engagement and Companionship The client needs a caregiver who will be present, attentive, conversational, and genuinely engaged. Meaningful companionship is an important part of this assignment. The caregiver should: Initiate friendly and appropriate conversations Offer reassurance, encouragement, and emotional support Participate in activities the client enjoys Help maintain a calm, positive, and familiar daily routine Recognize when the client may be lonely, anxious, uncomfortable, or withdrawn Encourage safe participation in daily activities whenever appropriate Respect the client’s choices, preferences, personal space, and independence Remain patient if the client needs additional time, repetition, or reassurance Provide companionship without becoming distracted by a personal phone, television, or unrelated activities We are looking for someone who understands that quality caregiving includes both physical assistance and meaningful human connection. Meal Preparation, Snacks, and Hydration Depending on the client’s care plan and preferences, responsibilities may include: Preparing simple meals and snacks Encouraging appropriate food and fluid intake Providing hydration reminders Assisting with meals when required Cleaning food-preparation areas after use Washing dishes and returning items to their proper locations Observing and reporting appetite changes, swallowing concerns, nausea, or refusal of food or fluids Following all dietary instructions listed in the client’s care plan Attention to cleanliness, nutrition, hydration, and the client’s comfort is essential. Light Housekeeping and Home Support The caregiver will help maintain a clean, organized, comfortable, and hazard-free living environment. Responsibilities may include: Tidying commonly used areas Washing and putting away dishes Completing light laundry Changing bed linens when needed Wiping down frequently used surfaces Keeping the bathroom and personal-care areas clean Removing trash Returning supplies and mobility equipment to their proper locations Keeping floors and walking paths free from fall hazards Reporting low supplies, maintenance concerns, or household safety hazards The client’s direct care, supervision, and safety must always remain the caregiver’s first priority. Observe, Document, and Report Changes The caregiver must closely observe the client’s overall condition and promptly report concerns to Helping Hearts. This includes changes involving: Mobility, balance, or transfer ability Physical strength or coordination Skin condition, bruising, redness, swelling, or pain Appetite or fluid intake Toileting patterns Mood, behavior, or cognition Alertness, confusion, agitation, or unusual behavior Breathing, comfort, or general condition The safety or cleanliness of the home Supplies that are low or unavailable Any fall, near fall, injury, or other incident Caregivers must complete accurate shift documentation, electronic visit verification, and required care notes through the company’s approved system. Concerns must be reported immediately. Caregivers must not wait until the end of the shift to report an urgent change, fall, near fall, injury, or safety concern. Small Dog in the Home The client has a small dog. Applicants must: Be comfortable entering and working in a home with a small dog Remain aware of the dog’s location when the client is walking or transferring Help prevent the dog, toys, food bowls, or leashes from becoming fall hazards Follow the client’s instructions regarding appropriate interaction with the dog Never allow the pet to interfere with the client’s care, supervision, or safety Routine pet care is not automatically part of the assignment unless it is specifically included in the client’s approved care plan. What We Are Looking For Exceptional Reliability and Continuity of Care This assignment requires a caregiver who can consistently work: Every Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Applicants should apply only if they can reliably commit to all three scheduled days. Regular attendance, punctuality, and dependable transportation are essential. Frequent call-offs, late arrivals, early departures, or ongoing schedule changes would disrupt the client’s care, safety, and established routine. JOB RESPONSIBILITIES The selected caregiver must be comfortable performing the following duties as authorized by the client’s care plan and Helping Hearts policy: Companionship and Active Engagement Remain genuinely engaged with the client throughout the shift through conversation, appropriate activities, encouragement, social interaction, and participation in the client’s normal daily routine. Engagement means interacting with the client rather than simply being physically present in the room. Personal Care Assistance Provide respectful assistance with activities of daily living while maintaining the client’s privacy, dignity, comfort, and independence. This may include bathing, showering, grooming, oral hygiene, dressing, toileting, hygiene assistance, and incontinence care when required. Bathing and Shower Assistance Help prepare for and safely complete bathing or showering. Remain attentive to balance, weakness, dizziness, slipping hazards, and fall prevention throughout personal care activities. Dressing and Grooming Assist with clothing, shoes, fasteners, hair care, oral hygiene, and other routine grooming needs while encouraging the client to safely participate as much as possible. Toileting and Bathroom Assistance Provide safe assistance to and from the bathroom, help with clothing management and hygiene, and remain available when the client requires supervision or assistance. Bathroom safety is an important part of this assignment. Mobility and Ambulation Assistance Provide the level of walking or mobility assistance identified in the client’s care plan. This may include standby assistance, walking beside the client, and assisting with approved mobility equipment. Transfer Assistance Assist with approved transfers within the caregiver’s training and according to the client’s care plan. Proper body mechanics and safety procedures must always be followed. Caregivers must never attempt a transfer that cannot be completed safely. Fall Prevention and Safety Monitoring Maintain awareness of the client’s environment, walking surfaces, mobility, balance, footwear, and potential hazards. Daily Routine Support Help maintain a consistent and positive daily routine involving personal care, meals, hydration, activity, companionship, movement, and appropriate rest. Activities and Socialization Encourage activities appropriate to the client’s interests and abilities. This may include conversation, games, reading, music, television programs, appropriate movement, or participation in approved activities. Light Housekeeping Maintain a clean and safe client environment. Duties may include cleaning up after meals, washing dishes, wiping counters, straightening the client’s living area, keeping the bathroom tidy, removing trash, making the bed, and maintaining clear walking pathways. Client Laundry and Linens Complete client related laundry when needed, including washing, drying, folding, putting away clothing and linens, and changing bed linens as appropriate. Environmental Safety Observe for spills, clutter, blocked pathways, unsafe furniture placement, misplaced equipment, or other hazards that could increase the client’s risk of injury. Observation of Changes in Condition Remain highly observant throughout the shift. Report significant changes involving mobility, balance, weakness, dizziness, alertness, behavior, appetite, hydration, bathroom habits, communication, energy level, or overall condition. When an update is requested, caregivers must provide a meaningful written response rather than a one word answer. Professional Boundaries Maintain appropriate boundaries with the client and family. Scheduling, pay, additional hours, time off, and employment related matters must be handled through Helping Hearts Operations. Shift Handoff Provide an appropriate handoff to an incoming caregiver when applicable, including relevant observations, safety concerns, changes in routine, and unfinished care needs. Full Shift Commitment Remain for the complete scheduled 7:00 AM to 7:00 PM shift unless released by Helping Hearts Operations. Emergency Response Remain calm, protect the client from additional harm, contact emergency services when appropriate, follow Helping Hearts emergency procedures, and notify Operations promptly. REQUIRED SKILLS The strongest candidate will have demonstrated experience with personal care, bathing assistance, toileting support, safe mobility assistance, fall prevention, client supervision, meal preparation, hydration encouragement, companionship, recognizing changes from baseline, professional documentation, and communicating concerns appropriately. We also need someone with strong situational awareness and judgment . You should know when to provide assistance, when to encourage independence, when something does not appear normal, and when it is time to immediately contact Operations. Assignments are matched according to the caregiver’s experience, skills, availability, location and ability to safely complete the care plan. Companionship and Household Support Some clients may primarily need: Meaningful conversation and companionship Social engagement and recreational activities Meal preparation and cleanup Grocery organization and errands Light housekeeping Laundry and linen changes Organization of the client’s immediate living space Medication reminders Transportation to approved appointments and activities Safety observation and general supervision Personal Care and Activities of Daily Living Other clients may require hands-on assistance with: Bathing or showering Sponge baths and personal hygiene Dressing and undressing Grooming, hair care and oral care Toileting and incontinence care Changing protective undergarments Meal assistance and hydration reminders Walking and mobility assistance Wheelchair support Entering and exiting vehicles Repositioning and comfort measures Maintaining dignity, privacy and independence during personal care Dementia and Memory-Care Support Some assignments may involve clients living with dementia, Alzheimer’s disease or other cognitive conditions. Responsibilities may include: Providing calm and consistent supervision Offering appropriate redirection and reassurance Supporting established routines Monitoring for confusion, agitation, wandering or changes in behavior Encouraging safe and appropriate activities Maintaining a quiet, respectful and supportive environment Following client-specific communication and safety strategies Immediately reporting significant behavioral or cognitive changes Caregivers assigned to dementia or memory-care clients must remain patient, attentive and aware of the client’s location and safety throughout the entire shift. Mobility, Transfer and Higher-Assistance Support Some clients may have limited mobility, be at high risk for falls or require significant physical assistance. Depending on the care plan and caregiver training, responsibilities may include: Assisting with walking and balance Supporting wheelchair mobility Assisting with bed mobility and repositioning Helping clients move between the bed, wheelchair, chair, commode or toilet Using an approved gait belt when directed Assisting with sit-to-stand or stand-to-sit transfers Using a mechanical or Hoyer lift only after required training and authorization Following all one-person or two-person transfer requirements Requesting assistance when a transfer cannot be completed safely Caregivers must never attempt an unsafe transfer or use equipment they have not been trained and authorized to operate. Non-Medical Health Support Depending on the assignment, caregivers may also be responsible for: Providing medication reminders according to the approved care plan Assisting clients with approved blood-glucose monitoring Documenting readings when required Observing food and fluid intake Monitoring for changes in physical, emotional or behavioral condition Reporting falls, injuries, pain, swelling, bruising, redness, skin concerns, confusion or unusual behavior Following established emergency and office-notification procedures Accurately documenting all completed tasks, observations and concerns Helping Hearts provides non-medical personal support. Caregivers must remain within their authorized role and follow the client’s approved care plan. Physical Requirements Applicants must be able to safely perform the essential responsibilities of their accepted assignment, with or without a reasonable accommodation. Physical responsibilities may include: Standing, walking or sitting for extended periods Frequently bending, reaching, turning and twisting Kneeling or squatting when necessary for safe care Using stairs in certain client homes Assisting clients with walking, balance and mobility Pushing or maneuvering a wheelchair Assisting with repositioning in a bed or chair Supporting safe transfers according to the care plan Handling laundry, groceries, household supplies and care equipment Completing light housekeeping throughout the shift Wearing required personal protective equipment Responding promptly during falls, emergencies or changes in condition The physical demands of each assignment may vary. Client Environments Care may be provided in: Private homes Independent-living communities Assisted-living communities Memory-care communities Senior-living facilities Other company-approved settings Applicants should understand that: Some homes may have dogs, cats or other household pets. Some clients require caregivers who are non-smokers. Some client environments may not be completely smoke-free. Applicants must disclose allergies, pet restrictions, smoke sensitivities or other environmental limitations before accepting an assignment. Family members, facility employees or other approved individuals may be present. Approved safety cameras or monitoring equipment may be present. Caregivers must follow all facility, household, confidentiality and safety requirements. Transportation, parking and travel conditions will vary by location. Caregivers must respect the client’s home, property, privacy and personal preferences. Hours and Assignment Disclaimer Helping Hearts offers day shifts ranging from 4 to 12 hours. Specific hours and assignments depend on: Current client needs The client’s requested schedule The client’s care level The client’s location Caregiver experience and qualifications Caregiver availability Transportation and travel distance Client and family preferences Assignment compatibility Changes in the client’s condition or service needs Full-time or part-time status does not guarantee a specific client, assignment, schedule or number of weekly hours. Client schedules may increase, decrease, change or end based on the client’s needs and preferences. Clients and families may request a different caregiver when they believe another caregiver would be a better fit. Caregivers may be considered for additional assignments when their availability, experience and location match current needs. Once a caregiver accepts an assignment, dependable attendance and completion of the entire scheduled shift are required. Employment Qualifications Applicants must: Be at least 18 years old. Possess a high school diploma, GED or equivalent. Have professional caregiving, personal-care, home-care or related experience. Demonstrate compassion, patience, professionalism and dependability. Follow written care plans and office instructions. Communicate professionally with clients, office staff and approved facility personnel. Complete accurate electronic documentation during every shift. Have reliable access to a smartphone. Maintain client confidentiality. Remain attentive and awake throughout every assigned shift. Follow all attendance, call-off, clock-in, clock-out and documentation requirements. Safely perform the physical responsibilities of the accepted assignment. Complete required orientation, training and assignment-specific instruction. CPR and First Aid certification are not listed as pre-interview document requirements for this position. Transportation Requirements Applicants must have: A valid current driver’s license Active automobile insurance Reliable personal transportation An acceptable driving record The ability to travel to assigned client locations The ability to transport clients when required and authorized Caregivers must immediately report any change affecting their driver’s license, automobile insurance, vehicle availability or ability to transport clients safely. Background and Screening Requirements Applicants must: Complete a comprehensive background check covering up to 15 years, where legally permitted. Complete required abuse, neglect and sex-offender registry checks. Complete a pre-employment drug screening when applicable. Participate in random drug screening when required by company policy. Provide favorable and verifiable professional references. Report any criminal charge or legal change that may affect their ability to provide safe care. Have no disqualifying history related to abuse, neglect, exploitation, theft, fraud, violence, weapons or other offenses that would prevent safe employment, subject to applicable laws and individual review.
