Connecticut Children’s is committed to providing a safe environment for patients, visitors and employees. The Patient Care Partner promotes the safety, health and comfort of patients, families and staff as related to the care of behavioral health/ psychiatry patients in the hospital. The Patient Care Partner may be assigned to Emergency Department or inpatient units to meet the unique needs of this population of patients. The Patient Care Partner is a staff member who has been trained to visually monitor a patient and provide direct patient contact or care. The Patient Care Partner may be assigned to the following patients: Homicide attempt Positive screen with suicide assessment or suicide attempt Violent behavior towards self or others Elopement/ wander risk Fall risk Confused mental state/ cognitive impairment Patients with eating disorders (anorexia, bulimia) Responsibilities include, but are not limited to: Patient Care: Receives hand off from the care team to include reason for observation, patient behaviors and any concerns/ risks. Provides hand off at the end of assignment. Notifies the primary nurse of changes in patient behavior. Collaborate with patient care nursing staff regarding all aspects involved with the patient being observed. Communicate effectively with the patient nursing staff regarding observed patient behaviors and all changes. Understands the observation levels and adheres to the standards for the assigned role (constant observation, 1:1, roving attendant/ every 15 minute checks) Provide direct, face-to-face, observation of patients at risk for harming themselves or others and/or for patients placed in restraint and/or seclusion. Incorporate de-escalation techniques in daily practice with patients being observed as needed. Supports restraint activities as delegated by the primary Registered Nurse. Assists with procedural holds as directed by MD, Provider and/or RN. REQUIRED WORK UNIFORM Helping Hearts USA LLC caregivers are expected to arrive for every shift clean, properly groomed, and professionally dressed. Required Uniform: Solid Navy Blue Scrubs Caregivers must wear: Clean, solid navy blue scrub tops and scrub pants. Closed-toe and closed-heel shoes appropriate for caregiving duties and transfers. Clean, secure, professional footwear. Crocs, open-back shoes, slippers, sandals, flip-flops, or other inappropriate footwear are not permitted. Hair must be clean, neat, and appropriately secured when necessary for safe hands-on care. Personal hygiene must be maintained throughout every shift. Strong perfumes, colognes, body sprays, and fragrances should be avoided because clients may have allergies, sensitivities, or respiratory conditions. Caregivers must remain appropriately dressed throughout the entire shift. A client's home is a professional workplace while you are on duty. ADDITIONAL EMPLOYMENT REQUIREMENTS Applicants must: Have verifiable hands-on caregiving experience. Have previous hands-on Hoyer lift experience. Provide verifiable professional employment references. Demonstrate a history of reliability and job commitment. Have a valid driver's license. Have reliable transportation to and from assigned client locations. Provide proof of current automobile insurance when required. Successfully complete required criminal background screening. Meet applicable abuse-registry and sex-offender-registry screening requirements. Complete required drug screening when requested and remain eligible for random drug screening according to agency policy. Possess or obtain current CPR and First Aid certification as required. Provide required identification and employment documentation. Provide a high school diploma or GED as required. Provide a complete five-year employment history when requested. Successfully complete agency onboarding and orientation. Complete all required agency training. Complete client-specific training and competency verification when required before independent placement. Be able to accurately use the agency's electronic scheduling, clock-in/clock-out, documentation, and communication systems. Maintain client confidentiality and appropriate professional boundaries at all times. Maintain reliable communication with management.
