JOB PURPOSE: Responsible for assisting residents with medications ordered by their appropriately licensed health care providers and as established in each residents individualized care plan. May provide medication reminders, assistance with self-administration of medications and administration of medications as allowed by state and federal laws and applicable rules according to company policies and procedures. Key communicator with the resident’s pharmacy, healthcare provider and family regarding medication management. Follows proper medication storage and disposal practices Recognizes possible medication side effects or possible adverse reactions and takes appropriate actions Knows which medications unlicensed staff may never provide assistance with, administer or provide supervision for and never does so. Monitors resident’s medication quantities and related supplies and makes arrangements for timely procurement of refills so resident dose not miss any doses or required monitoring. Makes arrangements for timely procurement of newly ordered medications. Makes appropriate physician & family contacts in a professional manner when order clarification is needed, regarding resident reactions to medications or other medication concerns. Accurately documents assistance/administration and resident refusal of medications according to policy and professional standards. Follows policy, professional standards, and the law regarding storage, assistance/administration of, documentation and disposal of controlled medications Immediately reports and documents any medication errors to the administrator or his/her designee per policy and takes appropriate actions for resident’s well-being. Reports and documents any resident change in condition. Uses good judgment in seeking necessary assistance from peers, supervisor, administrator, physician, family and EMS as needed. Obtain accurate pulse and respiration rate; obtains accurate blood pressure, body temperature, and blood glucose reading Accurately documents the effectiveness of medications given on an as needed basis and does so in a timely manner. Monitors medications for accountability by verifying medications have been ordered appropriately, are received as ordered, and controlled med drug counts are done at change of shift. Communicates effectively from shift to shift & to others (family, pharmacy, physician etc.) as necessary to facilitate the accuracy of any needed follow-up regarding resident medications. Routinely inspects, cleans and monitors the med storage system, equipment and supplies for resident safety. Reports any concerns to the supervisor for correction or repair. Assists direct care partners with their duties as part of the direct care team Completes thorough face to face change of shift report to CMA relieving for next shift. Performs other duties as assigned that are within the scope of the CMAs practice, training, and abilities. At discretion of charge nurse, count narcotics, give all routine prescribed narcotics, and document same on individual sheet and medication sheet. Qualifications Must have successfully completed state approved school of medication administration. Must carry current acknowledgment card or certification issued by state agency. Responsibilities Key Responsibilities: Work under the supervision of a licensed vocational nurse or a registered professional nurse to perform the following duties: After authorization by the facility’s licensed nurse or the resident’s treating physician, administers PRN medications. Organizes assigned medication duties to pour, pass, and document all routine prescribed medications and to give PRN medication only upon the instruction of the licensed nurse. Must document all PRN’s according to State Standards in all areas of the chart. Observe and report to the facility’s charge licensed nurse reactions and side effects of medication commonly administered to nursing facility’s residents. Take and record vital signs prior to administration of medications which could affect or change the vital signs. Administer and document regularly prescribed medications which the medication aide is permitted to administer only after personally preparing (setting up) of those medications to be administered. Document the medications given in the resident’s clinical record. Administer oxygen per nasal channel or a non-sealing face mask only in an emergency. Immediately after the emergency, the medication aide shall verbally notify the licensed nurse on duty or on call and appropriately document the activity and notification. At discretion of charge nurse, count narcotics, give all routine prescribed narcotics, and document same on individual sheet and medication sheet. Qualifications Must have successfully completed state approved school of medication administration. Must carry current acknowledgment card or certification issued by state agency. Review, read, notate, and initial the electronic health record to document and learn about pertinent information about residents. Receive medication updates from Resident Care Director (RCD) or Wellness Nurse. Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass (“Right” resident, medication, dosage, time, route, right to refuse). Ensure that medications are passed according to times and utilizing the mobile med cart. Document and initial as medications are given and ensure that appropriate documentation is completed for refusal or missed doses. Maintain confidentiality of all resident information including resident medication among other residents. Report all resident concerns and unavailable medications, made while administering the medication, to the RCD or Wellness Nurse. Restock medication cart after all medication passes. Assist in checking medication regardless of packaging system. Assess the residents to determine need for “as needed medication” and appropriately document and report to supervisor. Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies. Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications. Follow re-fill process for medications and notify RCD or Wellness Nurse if a new prescription is needed. Help residents maintain independence and promote dignity and physical safety of each resident adhering to the Sunrise Principles of Service. Strive to understand and respond to each resident with empathy, always remaining mindful of the resident’s unique communication patterns, history, and basic human needs. Practice routinely good standard care precautions of cleanliness, hygiene, and health. Resident Care Communications Notify RCD of any resident and/or family concerns. Attend and actively participates in daily Cross Over meetings facilitated by the LCM. Participate in the development of the Individualized Service Plans (ISP). Risk Management and General Safety Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations. Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures. Report all accidents/incidents immediately. Report all unsafe and hazardous conditions/equipment immediately. Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits and in compliance with fire codes. Comply with all infection control techniques, placement of bio-hazard containers, removal techniques, procedures, and policies. Understand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials. Report occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately. Ensure oxygen tanks are stored safely, exchange guest/resident’s tanks when empty, and monitor to make sure liters of oxygen are at prescribed levels.
